I. Statement of Violation
" Criterion V of Appendix.B to 10 CFR 50 requires thatinctivities affecting quality be prescribed by and accomplished in accordance with-appropriate instructions, procedures ~, or drawings. This requirement is.
~
nplified by ' the approved Quality Assurance Program Description. (QAPD) of South Texas Project (STP).
" Standard Site Procedure (SSP) 9, Revision 4, paragraph 5.6.6'.5, requires that the actual cold load position of spring hangers be' maintained after
.
balancing a piping system:in accordance with the procedure as outlined'in Appendix 2 of Specification'5L340JS1002 and Attachment SSP 9-42.
" Contrary to the above,.the construction support group. failed to maintain i
the cold position of a spring hanger (RC-1212-HL-5001) after balancing of i
the piping system by not assuring the line was filled with water prior to
.
verifying the cold load position."
II. Reason for Violation
J This violation occurred due to inadequate interface.between Startup and Construction.
Startup did not inform Construction that spring hangers, reworked after initial balancing, required resetting.
In addition, Construction engineers did not ensure that piping.was filled with water when reworked spring hangers were reset.
III. Corrective Action Taken and Results Achieved In response to this violation, STP conducted a review of the safety related spring hangers in Unit.1.
STP determined that fifty-five (55).
spring hangers were reworked after initial balancing of their.ascociated piping systems. Of these 55, STP could not confirm that nineteen (19)
had been properly reset, o
STP determined after further review, including in some cases filling
'
of the associated piping, that eleven (11) of these hangers vere set
!
satisfactorily and did not require resetting.

o STP determined that five (5) hangers required resetting.

IV. Corrective Steps Taken to Prevent Recurrence STP revised the Standard Site Procedure (SSP-9) for pipe support I
installation to include specific direction for Construction ), document at the time of balancing that the piping associated with a spring hanger is filled, if required.
To improve the interface between Construction and Startup, appropriate group personnel were reinstructed in the requirements for resetting spring hangers when they have been reworked after initial piping system balancing.
V. Date of Full Compliance STP is in full compliance with site procedural requirements at this time.
Appropriate setting of the three (3) remaining Unit 1 hangers will be confirmed prior to entering into Mode 3.
