Results Of the four areas inspected, no items of noncompliance or deviations were iden-tified.

Exit Interview The inspection scope and findings were summarized on November 29, 1979, with those persons indicated in Paragraph 1 above.

The inspector addressed the two inspector follow-up items in detail.

The licensee stated that the items would be reviewed.

Item (400/401/79-23-05),

Defective hanger welds -

The inspector reviewed the status of the reinspection of hanger welds which was initiated by the licensee's Deficiency and Disposition Report (DDR) No. 316.

(This item was reported to RII as a potential construction deficiency report on November 2, 1979.)

At the time of this inspection the licensee had completed the majority of the reinspection effort with the only outstanding items being hanger welds that are currently inaccessible.

This item will remain open pending completion of the licensee's reinspection and evaluation of the proble "2-There were no items of noncompliance or deviations identified in this area of inspection.

Independent Inspection (Units 1, 2, 3 and 4)

The inspector reviewed the licensee's program for visual inspection-of piping welds, particularly socket welds, and discussed the necessity for providing adequate guidance, within the procedure, to preclude inspectors selecting the wrong acceptance criteria for weld size.

(i.e.,

1.09t for socket weld fitting except flanges which are 1.4t)

The discussion also covered the necessity for providing assurance that pipe withdrawal on socket connections had been inspected.

The area of socket weld inspection will be reviewed in'a subsequent inspection and was identified as inspector follow-up item no.

50-400, 401/79-26-01 and 50-402, 403/79-25-01, QA/QC inspection procedures.

There were no items of noncompliance or deviations identified in this area of inspection.

Steel Structures and Supports - Observation of Welding Activities Within Containment (Unit 1)

The containment liner is being erected by Chicago Bridge and Iron (CBI) in accordance with ASME Section III Division 2, 1975 Edition through Winter 1975 Addenda as specified in the CBI QA manual for this contract.

The inspector observed the following welding activities during this inspection:

a.

Welding of studs onto sections of the dome liner plate in accordance with CBI General Welding Procedure GWPs - Stud, Rev.

The inspector observed the preparation of the surface for the stud welding operations and the inspection of the final weld.

b.

Ultrasonic inspection for laminations of areas on dome plate segments nos.

70 and 71 where reinforcement plates were to be welded.

c.

Welding of reinforcement plates on dome plate segments nos.

70 and 71.

d.

Welding of leak chase channel on plate segment nos.

70 and 71.

e.

Ieak chase test on plate no. 71.

f.

Welding of horizontal seam between rings 15 and 16 on the containment.

g.

Spot radiography of one foot section of horizontal seam between rings 15 and 16.

The welding and inspection activities were inspected for compliance with the procedures provided in the CBI QA Manual for this contract.

There were no items of noncompliance or deviations identified in this area of the inspectio During a part of this inspection the inspector was accompanied by a member of the licensee's QA/QC welding inspection group who was purportedly con-ducting a routine surveillance inspection of the CBI activities.

When asked what procedure was being followed for this surveillance activity the licensee was unable to provide one.

Further pursuit of this matter showed that the licensee's QA program provides for special qualification of personnel for containment liner erection monitor assignments but does not provide a

procedure for the "monitor" to work to nor does it provide for a frequency of inspection.

The licensee indicated during the discussion about this subject that CBI provides the QA and QC on the containment liner erection job and that the licensee's activities were in addition to the requirements and therefore, required no procedure.

The inspector stated that this item would be followed up on a subsequent inspection and would be identified as inspector followup item nos.

50-400, 401/79-26-02 and 50-402, 403/79-25-02, QA surveillance of contractor welding activities.

Steel Structures and Supports - Visual Examination of Welds The inspector visually examined the seam welds in dome plate segments nos.

70 and 71 for compliance with the requirements of the CBI Manual.

Attributes inspected included reinforcement, contour, undercut and workmanship.

There were no items of noncompliance or deviations identified in this area of inspection.
