a.
(Closed) Item 395/80-16-01:
Procurement of pipe to Regulatory Guide 1.66.
On April 14, 1980, the licensee reported a potential reportable item involving procurement of pipe to Regulatory Guide 1.66 requirements.
The licensee reported in a letter to RII on May 19, 1980 that the pipe shipped did receive the proper NDE requirements of Regulatory Guide 1.66.
The licensee reported that no safety implications are associated with this problem. This item is closed.

(0 pen) Item 395/80-16-02: RHR valve failure.
On April 22, 1980 the licensee reported an item regarding the failure of the "A" train RHR heat exchanger discharge flow control valve.
On May 19, 1980 the licensee issued a final report on this matter. Per the final report valve failure was due to site manufacturer of replacement pieces without design or QA program requirement considerations. Discussions with Nuclear Operations maintenance personnel and supervision indicate that new information on the RHR valve failure points to other causes for failure. Actions to correct the RHR valve failure are continuing.
This item will remain open pending completion of licensee actions.
c.
(Closed) Item 395/80-16-03: Machined fitting failure on Colt diesel.
On April 22, 1980, the licensee reported a substantial safety hazard under 10 CFR Part 21 involving the failure of a adapter for the Colt emergency diesel. On May 9, 1980, the licensee issued a final report on this item. The inspector reviewed the nonconformance notice (NCN)
- 65 written to assure correction of the adapter problem. Actions to correct the adapter failure have been completed.
The licensee is taking action to assure that Colt is informed on the evaluation of this item as a 10 CFR Part 21 item. This item is closed.

Failure of chiller unit.
On April 30, 1980, the licensee reported a deficiency in the "B" York centrifugal chiller unit. On May 29, 1980, the licensee issued an interim report on this matter.
This item will remain open pending receipt of the licensee's final report.

(Closed) Item 395/80-16-05:
Colt diesel generator crankshaf t thrust bearing clearances. On May 7,1980 the licensee reported a clearance problem between crankshaft thrust bearing halves on two Colt diesel generators.
On May 29, 1980 the licensee issued a final report on this matter which the licensee evaluated as being a substantial safety hazard as defined by 10 CFR Part 21.
The inspector reviewed the licensees final report and corrective action for this item reported on NCN 68. All action is complete. This item is closed.

(Closed) Item 395/79-40-01:
Rejectable indications 'in pipe spool piece welds. On December 13, 1979, the licensee reported to RII a rejectable weld defect in Southwest Fabricating and Welding Company supplied pipe spool pieces. On January 14, 1980, the licensee issued a final report on this matter. The inspector reviewed licensee correc-tive actions (NCN 1173) for the rejectable weld defect. All corrective actions relative to the reported problem have been completed. This item is closed.

(Closed) Item 395/80-10-04: Deficient socket weld fitting installa-tions. On March 19, 1980, the licensee reported that 3000 lb. fittings were incorrectly installed in 6000 lb.
fitting applications.
On April 18, 1980, the licensee provided a final report on this matter.
The inspector reviewed NCN's 1375, 1454, 1484, and 1540 to assure that deficient fittings had been properly dispositioned.
The inspector also reviewed the licensee reported actions to prevent recurrence to assure adequate corrective action. This item is closed.

(Closed) Item 395/80-05-05: Deficient design in Bahnson ventilation dampers.
On December 18, 1979 and February 8, 1980 the licensee reported a Bahnson design problem in ventilation dampers.
On February 12, 1980, the licensee provided RII a final report on this matter. The licensee evaluated this item to be a substantial safety hazard as defined by 10 CFR Part 21.
The inspector reviewed start-up field reports (SFR) 1660, 1827, and construction work requests (CWR)
2405 to assure that corrective action have been completed for the deficient ventilation dampers. The licensee has completed modifica-tions to the dampers in order to correct the problem. This item is closed.

(Closed) Item 395/79-36-02: Terry turbine governor valve spring on emergency feed pump.
On October 17, 1979, the licensee reported a deficiency with regard to the linkage for the governor valve for the emergency feedwater pump turbine.
The licensee has evaluated this item as a substantial safety hazard as defined by 10 CFR Part 21.
The inspector reviewed start-up field report (SFR) 1743 and construction work request (CWR) 2058 which the licensee issued to document correction of the valve linkage problem. The licensee has completed a governor valve linkage spring retrofit for the Summer Plant.

results. This review revealed that drawing control deficiencies are being identified, corrected, and actions taken to prevent recurrence.

(Closed) Inspector Followup Item 395/79-36-05: Site control of signifi-cant deficiencies. The Group Manager Production Engineering reported that Revision 9 to Nuclear Engineering Procedure No. NE-123 has been issued to require tracking of significant deficiencies and suustantial safety hazards by a proper control document. The inspector reviewed the NE-123 revision and held discussions with the Group Manager Produc-tion Engineering on past reportable items. The Group Manager indicated that previous reportable items will be reviewed to assure proper control documents. This item is closed.

Status of Inspection and Enforcement Bulletins (IEB's)
(Closed) IEB 80-09: Hydramotor actuator deficiencies. On May 5, 1980, the licensee issued a letter in response to IEB 80-09. The licensee reported that no ITT General Controls hydramotor actuators are used in the Summer Plant. This item is closed.
