The inspection scope and findings were summarized on March 6, 1980, with those persons indicated in Paragraph I above.

Licensee Action on Previous Inspection Findings (0 pen) Infraction (395/79-35-02): Undersize socket welds. The inspec-a.

tor questioned the licensee regarding the status of this item. The

--

licensee stated that reinspection for identification of undersize socket welds was almost complete but that repair and final inspection of the undersize welds would take several more months. Some of the socket welds could not be reinspected as they were embedded in concrete.

The inspector was informed that this situation had been evaluated and that such welds would.be -acceptable because of the restraining action provided by the concrete.

The inspector reviewed this disposition on Nonconformance Notice 1146. This item will be reviewed further by NRC Region II in subsequent inspection.

(0 pen) Unresolved Item (395/79-35-04): QC inspector's work questioned because of reportedly bypassed inspections.

Information provided to the NRC inspector by the licensee indicates that the subject QC inspec-tor did not perform very many inspections on safety-related pipe

welds. A sample of the safety-related welds he inspected will be reviewed by NRC Region II in a subsequent inspection.

-2-(Open) Unresolved Item (395/79-35-05): Fitup for butt welds did rot c.

meet requiremeats and grinding to correct the misalignment resulted in underwall conditions. The inspector reviewed results of the licensee's reinspection of ASME Section III Class 3 butt welds for unacceptable misalignment and underwall conditions. A sample consisting of 200 welds had been examined by the licensee. The licensee indicated that three of these welds had possible unacceptable mismatch and another five appeared to have wall thicknesses below the specified minimum.

Reexamination of the apparently unsatisfactory welds was being performed to confirm the findings. The licensee informed the inspector that it was anticipated that work on these items would be completed in April 1980.

The data obtained and the licensee's evaluation of this data will be reviewed by NRC Region II in subsequent inspection.

(0 pen) Unresolved Item (395/79-35-14): Violations of high-low require-ments on difficult to reach welds. The inspector discussed this item with the licensee. This item appears related to item 395/79-35-05 (above) and the licensee indicated it will be examined in conjunction s

with that item. Region II will review the licensee's findings in subsequent inspection.

(Open) Unresolved Item (395/79-35-06): Small diameter non-code piping e.

used in ASME Section III Class 1 application. The inspector had previously requested a comparison of receipt versus use data for small diameter ASME Section III Class 1 piping, such that any installation of unaccounted for piping could be identified (Reference Region II Report 50-395/80-03). The inspector questioned the licensee relative to the status of this work.

It had not been started but was begun during the inspection. The licensee indicated it would be completed in less than a week. This information will be reviewed by Region II in subsequent inspection.

(Open) Unresolved Item (395/79-35-08): Underwall condition on a 6000#

reducer insert. The inspector reexamined the questioned inserts.

addition, material certifications were reviewed.The inspector found that the heat number recorded for installation of one insert was traceable not to certifications for 6000# inserts but rather to 3000#

inr2rts. The licensee noted that this had been identified in a recent surveil. lance and was being investigated. The NRC inspector reviewed documentation of this, finding in licensee QA Surveillance Report 3803 (dated 3/6/80). The liciensee subsequently informed the inspector that reexamination of radiographs of the inserts led them to believe the questioned insert was a 3000# rather than 6000# fitting.

It did, however, appear to meet the minimum wall requirements for a 6000#

insert. The licensee's findings relative to this item will be reviewed further by Region II in subsequent inspection.

Note:

This item is considered to be closely related to items 395/79-35-06 (above) and 395/79-35-13 (below), as the non-compliance (if confirmed) for all three items appears to stem from improper verification of material before instal-lation.

(Open) Unresolved Item (395/79-35-13): Underwall socket-welding 90*

elbows. The licensee informed the inspector that an underwall elbow had been identified in the Chemical Voluen Control System.

Specifically, a 3000# elbow was installed where a 6000# elbow had been required.

This finding was documented by the licensee on Nonconformance Notice 1375 (dated 2/12/80). The licensee stated that the reinspection of socket fillet weld size br-3 performed relative to item 395/79-35-02 would identify any other unuerwall fittings present as undersize fittings would result in undersize weld. This item will be examined further by Region II insubsequent inspection.

(Open) Unresolved Item (395/79-35-09): Pipe not properly backed out of sockets. The inspector questioned the licensee relative to the status of work on this item. The licensee ir.Iormed the inspector that 265 socket weld joints had been examined :adiographically and that the findings were being reviewed to determine their significance.

This item will be examined further by Region II in subsequent inspection.

(Closed) Unresolved item (395/79-35-11):

Improper welding on the incore pit liner. The inspector reviewed the report of a licensee audit relative to this item (Investigative Audit of Allegations Related to the Reactor Vessel Cavity Liner, dated 1/28/80) and discussed the safety significance of the liner with the resident engineer. The audit report provided some evidence of improper welding in that welders not properly qualified vare found to have welded on the structure.

The resident engineer,

witnessed and reviewed work on the liner when it was in prcgress sieted that he considered work on the liner to have been satisfactory. Further, he indicated that based on design considerations little structural ;trength was required of the liner and its safety significance was minimal.

Based on the apparent lack of safety significance, this item does not require further inspection and will be closed.

j." ' (Open) Unresolved Item (395/79-35-12): Alleged violations of sicket welding procedure requirements. The inspector questioned the licensee relative to this item and was informed that it was still under review.

Region II will address this item further in subsequent inspectior.

(Closed) Unresolved Item (395/79-35-03): Missing documentation of

flange traceability information and installation verification.

The inspector verified proper entries added to the Bill of Materials and examined the flange for acceptable identification. The requirements of licensee procedure WP-VII-09 were found to be met relative to this In examining work packages for other piping (including DE-RC-06C, area.

DE-SI-04 and DE-CS-02) no additional examples of missing entries were identified. This item is considered to have been an isolated occurrence and the item will be closed.
