Plant Tour The inspector observed work activities in progress, completed work and construction status in Unit 1 and 2.
Specific activities and completed work observed by the inspector included electrical wiring, cable trays and connections to electrical penetrations.

Unresolved Item (388/78-15-01):
Repair and storage of Unit I and
RHR Heat Exchangers.
These items had previously been reviewed during inspections 387/80-10; 387/80-21; 388/80-06 and 388/80-14 but remained unresolved pending engineering review of repair documentation and proper storage of the Unit 2 equipment.
The inspector examined the documentation shown below concerning repair and cleanup of the heat exchangers:

OC Inspection Reports (QCIR) Nos. 49.0-M1-1E-205-6 and 49.0-M1-2E-205-5 for RHR Heat Exchanger modification per GE letters EAG-275 and EAG-160 which provided retrofit procedures for RHR baffle. plates for Unit 1 and 2 heat exchangers.

QCIR No. 49.0-M1-1E-205-19 and 49.0-M1-2E-205-20 for RHR Heat Exchanger rework of impingement plates per GE FDDR KR1-050 and KR2-033 for Unit
- 1 and 2 heat exchangers. QCIR No. 49.0-Ml-lE-205-13 and 49.0-M1-2E-205-11 for cleaning of Unit 1 and
RHR Heat Exchangers per GE FDDR-KR1-044.

1443 to verify location of failed tube and plug six tubes per GE EAP-399 and Bechtel Procedure P 34-T-AG which was: closed July 21, 1978.
The inspector had no further questions concerning items 387/78-03-08, 387/78-16-03 and 388/78-03-02.
However, the inspector was informed that the licensee had decided to remove the chromate solution used. in lay-up of the Unit 2 heat exchanger and replace it with dry nitrogen.
The inspector examined.Bechtel Procedure FCl-M-193, Revision B, "Removal of Chromate Solution and Dry Nitro-gen Lay-up of Residual Heat Removal Heat Exchangers 2E-205 A and B".
Item 388/78-15-01 remains unresolved pending completion of the nitrogen lay-up.

Noncompliance (387/79-31-02):
Use of ice water to minimize interpass temperature during welding without provision for such use in welding procedure.
Paragraph 12.1.6 of FC1-M-174, Revision 4, provides for demineralized ice water cooling of the pipe being welded as identified by this item.
Bechtel 10M dated February 6, 1980, confirmed that prior to GE approval a
mock-up of this joint had been welded using the same procedure and cooling/
quenching method and had been found acceptable.
The inspector had no further questions concerning this item.

Field Procedure FP-P-11, Revision 4, dated April 17, 1980 which provided a detailed 'procedure for making and'ontrolling field changes to con-struction drawings and assuring that such changes will be incoroorated into the final field revision b.
License Audit No. 79, May 21-25, 1979, of "Bechtel Design Document Control" which included audit of control of,"red lined" (field change)
. drawings for revision of master reproducible.
This audit identified nine revisions not completed within the required two weeks.

Licensee unannounced re-audit (July 3, 1980) of "Bechtel Production Print Control and Bechtel Red Lining of Piping Drawings" which closed
'ut the nonconforming revisions of Audit No. 79 and found that control of red lined drawings,was acceptabl,e.

Cleanliness inspections of recircu-lation loop "B".
The inspector questioned compliance with GE Specification 22A-2537 due to lack of a "crawl-through" prior to hydrostatic testing.
GE letter EAG-1324'states that lack of a "crawl-through" inspection was not a violation of procedures.
This item is resolved.

Failure to document a nonconforming condi-
,tion identified by the gC inspector during weld repair GBB-104-1-FW16-Rl.
Fol-lowing identification of this item by the NRC inspector, the nonconforming condition was documented on Rework Notice No. 3892.
This item was dispositioned to bring the gap within tolerance and rework was completed on March 28, 1980.
The inspector also examined records of training sessions for ASME-III welding on March 27 and 28, 1980.
The inspector had no further questions concerning this item.

Problems Identified at Other Sites The licensee was informed of the deficiencies noted below which had been re-ported at other sites and requested to determine if the companies had provided any equipment to the sites.

Waste gas decay tanks fabricated by Industrial Piping Supply Company were reported to require extensive repair as a result of poor quality welding.
The inspector was informed that this company was not on the vendor list and had furnished no equipment to the site.

b.
Malfunction of a power operated relief valve supplied by Control Com-ponents, Inc.
had been reported.
The inspector was informed that this company had furnished no equipment to the site.
