Other craftsmen and technicians were interviewed as necessary when the in-spection interfaced with their wor.
Plant Tour'Unit 1 and

The inspector toured the Unit Nos.
1 and 2 reactor buildings and primary containments observing work in progress and completed installations.
He examined the condition of installed electrical wiring, cable trays, exposed electrical terminations, pipe supports, piping and structural welds, struc-tural steel connections, and equipment storage.
The inspector found 12.each, ER 308L welding rods in a pipe fitter "gang box" at elevation 645'n Unit No.
2 reactor building.
He also found 6 each, E-70S-2 welding rods lying in the open at elevation 779'n Unit No. 2.
The Bechtel Melding Standard, MFNC-1, Paragraph 6.9 requires that, "Indi-vidual welders shall keep their work areas clear of unauthorized or discarded filler materials."
The failure to properly handle welding fillermaterial is an item of noncompliance (388/80-14-01).

Licensee's Actions on 10 CFR Part 21 Items a.
Containment Gas Anal sis S stem In a letter to the NRC, dated April 3, 1980; Comsip, Incorporated iden-tified potential problems with the "dead heading" the pump.
The licen-see has issued Nonconformance Report No. 5900 describing the condition.
The Nonconformance. Report is awaiting disposition at this time.
The inspector noted that the "Hold Tags" were missing from the Unit No.
1 installation and that, although the Unit No.
2 gas monitor was not yet installed, the nonconformance report did not include it.
The licensee affixed new Nonconformance
"Hold Tags" to the Unit No.
panels and extended the Nonconformance Report to cover the Unit No.
2 panels.
b.
Jamesbur tlafer Seal Valves The licensee identified a leak through a bolt hole on a 3" valve,
. HBD-BT-0-86-049.
The licensee initiated a Part 21 report, Nonconform-'nce Reports 5243 and 5836, and NCARl-53.
The reports identified 20 valves manufactured by Jamesbury which will be hydrostatically tested to confirm their integrity.
To date, 15 valves have been tested, the original leaking valve returned to the manufacturer and 4 valves that are installed in Unit No.
1 are awaiting testin C.

, There are two distinct problems involved in this Part 21 report.
First,. the Agastat'relays experience premature time out due to oil having been introduced during manufacture.
This occurred during a limited time. period and the defective relays can be identified by a serial number/date code system..
This condition is identified on Nonconformance. Report 5624.
The current plan" is to replace the defective relays.
Second, the Agastat and GE relays experience excessive time out due to bellows set.
The problem can apparently be avoided by. periodic exer-cise of -the relay.
t1CAR-1-54 identifies this condition.
No decision has been made at this time whether to replace. the relays with more dependable timers or to require periodic exercising.
American Narnin and Ventilation (AMV Dam er llirin Defects A!<V notified the NRC on May 6, 1980 of a Part 21 report concerning wiring deficiencies in ventilation damper controls.
The licensee generated guali ty Action Request 8856-F-604 and Nonconformance Reports 5307 and 5914.
The repairs consist of replacing wiring terminations, visual inspection, and replacement of any identified defective wiring.
These activities are covered by a procedure supplied by A!W, dated tune 30, 1980.
The inspector examined 6'junction boxes both before and after repairs.
He observed the craftsmen repair a junction box and verified that the work was performed in accordance with the procedure, that the proper crimping tools were used, and that inspections were made.
During visual examinations of the 6 junction boxes, the inspector noted that non-tnsu1ated throat reducer bushing were used in a limited number of installations.
For example, approximately 3 out of 4 bush-ings used on junction boxes 1'7604A and B were of the insulated throat type.
Drawing E-49, Sheet l8, Revision 15, Paragraph 4.2.12, requires in part, that:
"...Al'1 other conduit bushings, chase nipples, flexible conduit shall be. insulated throat or insulated bushing type only."
The licensee's staff is evaluating the acceptability of using non-insul'ated throat reducer bushings-in this application.
This item'is unresolved pending completion of the foregoing evaluation and review by the NRC (387/80-21-01).
The above items remain open pending completion of the licensee's corrective action.

Discusses:"the results of an inspec-tion in which 75% of 123 beams surveyed exhibited various cope defects.
PP&L PLA-443 - PP8L to NRC notification that defective beam copes are considered to be a potential Construction Deficiency Report.
Bechtel CME-4664 - Internal Bechtel memorandum discussing the results of tests performed on Industrial Engineering kfor k beams.
PPSL PLA-492 - PPSL to NRC withdrawing coped beam deficiencies as a Construction Deficiency Report.
In addition to the foregoing correspondence, Nonconformance Report Nos.
4288, 5017, 5534, 5536 and 5545 were reviewed.
Report 4288 required that beams identified as deficient inside primary containment be repaired in accordance with drawing C-1187, Sheet 2', Revision 0.
The inspector ver-ified that selected dimensions of components and welds meet the drawing requirements for Unit No. 1.
He observed welding undercut on 3 beam shear clips in excess of AHS Code tolerances.
The licensee documented this condi-tion on Noncon'formance Report 6386.
Discussions with the civil engineering group disclosed that these shear clips are no longer structural members due to the repairs implemented under drawing C-1187.
However, the licensee
.stated that additional clip welds would be inspected to ensure this was an isolated deficiency.
The NRC "inspector visually examined several more shear clip welds and could not locate any other deficient welds.

PP8L letter to the NRC, PLA-412, stated that the following activities were accomplished to correct'he noncompliance:
(1)
The licensee performed an inspection of Industrial Engineering Work (IEM).'beams.
NCR 4288 documented the results and corrective actions required per drawing C-1187.
The NCR 4288 and repairs per drawing C-1187 were reviewed by the, NRC inspector.
(2)
The l.icensee reviewed drawings to identify steel beams that re-quired coping and were supplied by IEH.
guality Control inspected these beams.
The inspector.
reviewed NCR 5024 and Field Inspection Report Cl'6-6779, dated January 23, 1980, which documented the inspection.
(3)
The guality Surveillance Plan was to be revised to include coping/
re-entry cuts as a hold point for inspection.
A review of guality Sur vei llance Plan No. C-16, Revision 1, dated September 28, 1979, verified that a hold point inspection had been incorporated.
This item remains open pending completion of Nonconformance Report 4288.

Unresolved Item (387/78-03-08):
RHR Heat Exchanger Repair and Storage.
The licensee has filled Unit.No.
RHR heat exchangers with demineralized water in preparation for preoperational testing.
He has developed procedure M-187, Chemical
"Met" lay.-up for Residual Heat Removal Heat Exchangers 2E-205A and B for Unit No. 2.
The "A" heat exchanger has been filled and tested satisfactorily.
The "B" heat exchanger was involved in a chemical spill and fillingwas postponed pending receipt of more chemicals..
This item remains open pending completion of storage activities and revi.ew of final repair records.

Exit In'terview

The inspector held an exit interview with representatives of the licensee's staff, denoted in Paragraph 1, at the conclusion of the inspection on August
.22, 1980.
He discussed the scope and findings of the inspection.
The licensee acknowledged the inspector's statements and presented informa-tion concerning the apparent item of noncompliance relating to uncontrolled weld'iller materials.
He stated, that meetings have been held with the general foremen to emphasize stricter control at their level, numbers have been assigned to weld rod stub cans, and additional training employed.
