Licensee Preservice Ins ection PSI Audits The inspector reviewed selected licensee audit reports related to preservice inspection contractor activities at the Susquehanna site.

These were con-sidered with respect to areas audited, findings and method of close out of nonconforming items.

The following were included in the inspector's review:

Audit number 86, January 28-29, 1980 Audit number 86A, March 28, 1980 Audit number 90, April 28 to June 5,

1980 Audit number 90A, June 16-17, 1980 Audit number 95, August 11-18, 1980 The above listed audits of Nuclear Energy Services Incorporated (NES)

reported significant findings in the following areas:

Personnel certification Equipment calibration

Level III evaluation of examination results Calibration block identification and material certification Noncompliance with the PSI program plan Noncompliance with NDE implementing procedures The inspector found that earlier audit findings were closed out, with one exception, by subsequent audits at the NES home office in Danbury, Connecticut.

The exception to the above involved the close out of item A3 of audit 86.

Audit 86A was the close out vehicle and appeared to omit the part of the finding as it rel'ated to the visual inspection certification of an NES employee assigned to the Susquehanna site as the NES Site Supervisor.

Dis-cussions with licensee personnel indicated that, in hi.s capacity as site supervisor, the individual had no visual inspection responsibility at the Susquehanna

.site and, therefore, his certification was no longer considered relevant.

The inspector had no further questions concerning this matter.

The inspector noted that approximately 15 nonconforming items were disclosed by licensee audit number 95 which was conducted by a four member team of licensee representatives including a licensee consultant.

This in-depth audit is not yet closed and has resulted in a voluntary cessation of activi-ties by NES.

The licensee stated that PSI work will not resume pending satisfactory resolution to the nonconforming items reported by the audit

,team.

In addition, the licensee stated that subsequent in-depth audits of the PSI contractor will be conducted to assure compliance with applicable codes and requirements.

No items of noncompliance were identified.

Licensee Action on Previous Ins ection Findin s (Open)

Noncompliance (387/79-30-01):

Failure to apply design control to meet Section XI examination requirements.

The inspector reviewed the following:

Southwest Research Institute document number 17-5941(6)

dated August 22, 1980 Bechtel document entitled "Report of the ultrasonic examination of the Susquehanna Riser-to-Sweepolet Circumferential Field Weld Mock-up Capped Double Weld Supplement 1."

The reviewed documents discuss the problems associated with ultrasonic examination of the double welds in the recirculation system but offer no solutions.

This item remains open pending further licensee action.

Noncompliance (387/80-12-03):

NDE personnel certification.

The inspector reviewed records submitted by Nuclear Energy Services Incorpo-rated (NES) to the licensee in an attempt to verify that proper certifica-tion was accomplished to UT Level II, of the individual in question.

The following were included in the inspector's review:

Memo to file dated 6/30/80 from M. Stamm, NES Manager, Field Operations Revised certification record listing experience and training hours The memo served 'to verify that 60 hours6.944444e-4 days <br />0.0167 hours <br />9.920635e-5 weeks <br />2.283e-5 months <br /> of home study was credited as part of the required 80 hours9.259259e-4 days <br />0.0222 hours <br />1.322751e-4 weeks <br />3.044e-5 months <br /> of training.

The inspector noted that the certification record gave credit for 16 hour1.851852e-4 days <br />0.00444 hours <br />2.645503e-5 weeks <br />6.088e-6 months <br />s-of ISI training which was conducted in April 1980 and for 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br /> of ISI training which was conducted in June 1980.

The 20 hours2.314815e-4 days <br />0.00556 hours <br />3.306878e-5 weeks <br />7.61e-6 months <br /> of training were conducted subsequent to the UT Level II certification date of February 28, 1980.

The record indicates that the individual was not fully qualified for certification as UT Level II on February 28, 1980.

The inspector telephoned the licensee on October 2, 1980 and stated that this item will remain, open pending resolution of the above noted discrepancy.

(Open) Unresolved Item (387/80-09-01):

Incomplete preservice inspection program.

The licensee stated that revision three of the program plan arrived at the site on September 25, 1980 and represented the complete pro-gram.

This item remains open pending review by an NRC inspector of the revision.

Noncompliance (387/80-09-02):

Failure to follow UT procedure.

The inspector reviewed the following:

Calibration data sheet number 2771-13 dated 5/13/80 Calibration data sheet number 2771-12 dated 5/13/80 The data sheets represent reexamination of the residual heat removal system welds listed on voided data sheet 2771-1,and 2771-4.

The documents verify that the reexaminations were done in accordance with the applicable straight beam requirements of procedure 80A2771.

The inspector had no further questions concerning this ite.

Exit Interview The inspector met with the licensee representatives (denoted in paragraph 1)

at the conclusion of the inspection on September 25, 1980.

The inspector summarized the purpose and the scope of the inspection and the findings.

In addition to those denoted in paragraph 1, the NRC senior resident inspector, Mr.

R. Gallo, attended the exit meeting.
