Unresolved Item (387/79-14-05)

Deficiency Report Log Item 0092, cable installation in embedded raceways.

The licensee issued a stop work order on cable pulling until the requirements of their June 22, 1979 letter were complied with.

The inspector verified that the Prerequisite requirements of the June 22, 1979 letter were complied with by Bechtel.

(Closed) Infraction (387/79-11-01)

Failure to properly brush aluminum conductors immediately prior to installing crimp connectors.

The inspector verified, in discussions with the electrical craft termination 'personnel, that they have been re-instructed on the site requirements for electrical termination and read a

memo No.

187 on terminations dated April 24, 1979.

The inspector witnessed the installation of power terminations and verified that the electrical craft did follow the field instructions for making. termination.

Cable Instal 1 ation The inspector examined the cable. installation of conduit CIK033.

This was the fitst authori'zed cable pull in conduit since the stop work order was issued by the licensee.

Two additional cables were to be pulled into the conduit, but they were not able to be pulled through.

All cables were removed from the conduit in order to re-align the cable input and at that time a 12 conductor No.

AWG Cable No.

CKQG0304R was identified as being damaged.

The cable was put on NCR and a review of the problem is being performed by the licensee.

This matter is considered to be unresolved and will be examined during a subsequent inspection (387/79-23-01).

Electric Motors and Valve 0 erations 5.

The inspector witnessed the installation of the RHR motors and four safety related electric valve operators.

The inspector reviewed'he applicable work procedures and inspection pro-cedures in the following area for the. above components:

Receipt, storage, handling, placing and installation of components and inplace inspection procedures.

The inspector verified that the requirements of the applicable specifications, quality control inspection reports (gCIR s)

and field,instructions were complied with.

No items of noncompliance were identified.

ualit Control Bechtel's quality control inspector verified that the cables being pulled into conduits were in accordance with the require-ments of procedure E-49 entitled "Conduit and Cable Tray Notes and Details".

The installation of cables were inspected in accordance with the requi'rements of guality Control Instructions (OCI) E-1.0, E-l.l and E-2.0 entitled "Installation/Inspection of Class lE Raceway".

The inspector witnessed the. cable pull.,in conduit ClK033 and verified that the electrical-crafts were following the latest instructions. and that the.guality Control.Inspector was per-forming the in process inspection as required by the above gCI' No,items of noncompliance were identified except as stated in Paragraph 3 of this report.
