The licensee stated that the above documents are planned to be answered in their SAR response to the NRC question 040.2 which states the following:
"The staff requires that the fol'lowing qualification test program information be provided for all Class 1E equipment:"
Identification of Equipmerit-Equipment design specification requirements Test Plans Test Set-Up Test Procedures Acceptability Requirements Test Results Identification of Documents listed above The information requested above shall include'the listed groups of Class lE equipment.
This item is considered to be unresolved pending submittal of SAR amendments concering the above areas and will 'be examinei by-an HRC inspector during a subsequent inspection.

Licensee Action on Previous Ins ection Findin s (Closed) Deficiency item (387/78-12-09):
Motor Control Center, MCC 1B217, had not been incorporated into the site maintenance program.
The inspector reviewed revision 16 dated October 4, 1978 to Field Procedure G-11 (FP-G-ll) entitled, "Storage, Protection, Maintenance and Lay Up for Susquehanna Units 1 and 2," which requires a duplicate '-
review of the Daily Receiving-Blotter by the storage and maintenance function arid a cross-check by field guality Control to verify that safety-related equipment is incorporated into the maintenance control system upon receipt.
The inspector reviewed the Daily Receiving Blotter for January 2 - 10, 1979 and verified that the lead storage and maintenance engineer is documenting items listed on the Blotter.
It appears that the present system should correct the itme identified in report 78-12-0 ualit Assurance
- Electrical Im 1ementin Procedures The inspector reviewed the engineering procedures which established the requirements for the installation of Class 1E electrical and instrumentation cables entitled, "Field Cable Terminations Procedure (FP-E-8)," and the quality control instructions (gCI) E-5.0 and E-5.1, Revision 2 dated December 18, 1978 which identified what to inspect and the accept/reject inspection criteria.
During the review of the gCI E-5.0 and E-5.1 the inspector noted that the inspection verification of the use of controlled terminating tools was deleted from the gCI when revision 2 was issued.
The inspector also noted that certain activity descriptions of the gCI were coded in the inspection criteria column. of the gCI with the word NONE, Inspection Activity Code S.
The inspector was in-formed by Bechtel guality Control for item 2.5 of gCI E-5.0 and E-5.1, they were inspecting 100Ã.
This level of inspection was not identified on the gCI as is required by Bechtel Power Corporation Project Special Provisions Notice SF/PSP G6.3,'evision 2, Job No.
8856 dated December 14, 1977.
This document which is a supplementary requirement to Field Inspection Procedure 6-6 states in part:
"When inspect, witness, test or review activities are to be implemented by use of a sampling plan, the sample size and frequency shall be identified in the Project guality Control Instructions under Activity Description (Block 4) or,...."
The licensee is presently reviewing these procedures and has identified similar type of problems with the existing Ouality Control Inspection plans.
This matter is considered to be unresolved and will be examined by an NRC inspector during a subsequent inspection..

Instrumentation E ui ment - ACR E ui ment The inspector reviewed the status to date of the problems identified by the licensee notification of a 10 CFR 50.55(e) deficiency associated with the following; "Fabrication Damage to Wiring on Termination Module Connectors for ACR Equipment."

Further investigation by the licensee at the site of the ACR equip-ment has been documented in Bechtel memorandum dated January 2,
1979.
This interim report identified additional problems other.
than the termination modules located in the termination cabinet 1TC621.
These problems were examined by the NRC inspector during this inspection period and are as follows:
Numerous insulation damages (cuts, nicks, and shield solder connections, etc.) were found in the non-safety portion of the equipment.
The safety related portion of the equipment will also be inspected following inspection completion of the non-safety portion of the equipment.
Interim report MCAR 1-29, Analysis of Safety Implications, states in part,
"Since the occurrence of damaged insulation is a random thing and could occur in any and all safety related circuits, the possibility of a common mode failure exists".
Cable k'4108/C94A-712 was found to have (4) conductors pulled out of its pin connectors.
It appeared that the tool used to lock the conductor into the pin connector did not perform a uniform swaging operation.
One set of pins had four identical swaged indents approximately 90'part, while other pins had three swaged indents and the barrel of these pins were not concentric.
It appears that the quality control inspection method used at the manufacturing site was not sufficient to identify these types of defects prior to shipment to the field site.
The planned corrective action approach as of this inspection period is to identify the problem areas by inspection and provide a re-work/inspection/test criteria to correct Unit
PGCC/ACR-system.
The plans for Unit 2 will also be reviewed by the licensee to assure that the condition of the Unit
PGCC system is not carried over into the Unit 2 system.
This item is considered to be unresolved.

Exit Interview The inspector met with the licensee representatives (denoted in paragraph 1) at the conclusion of the.inspection.
The inspector summarized the purpose and scope of the findings.
The licensee acknowledged the inspector's findings.
