As discussed with your Mr. G. E. Wuller on July 14, 1987, your response fails to address the deficiency that allowed Fire Penetration Seal III-A0-155 to be removed from service without initiating and/or maintaining the required Technical Specification fire watch surveillance.
Please provide information on the apparent procedural or programmatic deficiency that allowed this condition to occur.
Please provide the supplemental information within 30 days of the date of this letter.

'n LP&L Response to Violation No. 8710-02 VIOLATION NO. 8710-02 Failure to Adhere to T.S. Required Action for Inoperable Fire Penetration Seal Technical Specification (TS) 3.7.11 requires, in part, that when fire. rated sealing devices separating safety-related fire areas are inoperable. within I hour, a continuous fire watch must be established on at least or.e side of the affected device, or the operability of the fire detectors on at least one side must be verified and an hourly fire watch patrol must be established. Fire Penetration Seal III-AO-155 is a fire-rated sealing device separating safety-related fire areas.
Contrary to the above, prior to April 17, 1987, maintenance was performed

on Fire Penetration Seal III-AO-155 causing this seal to be in an inoperable condition. During the period of April 17 through May 1, 1987, no continuous fire watch was established or fire watch patrol tours performed for this inoperable seal.
This is a Severity Level IV violation.
RESPONSE (1) Reason For The Violation The violation is attributed to the fact that an initial evaluation was conducted by personnel not familiar with fire seal installation and inspection practices who determined that the fire seal appeared to be operable. Upon followup by the NRC Inspector concerning the quality verification of the fire seal installation, a more detailed inspection was performed by the appropriate Plant Maintenance personnel. During this inspection it was determined that the fire seal was inoperable.
(2) Corrective Action That Has Been Taken The fire seal in question (III A-0155) was evaluated on May 1, 1987 by Electrical Maint'enknee personnel at the direction of the Fire Protection and Safety Department and'upon removal of the damming t

material was determined to be degraded and therefore inoperable.
CIWA-032865 was generated to rework the seal. Additionally, Fire Appliance Impairment Form 87-093 was initiated and an hourly firewatch patrol conducted per T.S. 3.7.11.
The fire seal was subsequently repaired and dpciated operable on 5/8/87 at 1401 hours0.0162 days <br />0.389 hours <br />0.00232 weeks <br />5.330805e-4 months <br />.
(3) Corrective Action To Be Taken

A meno will be issued to Operations and Maintenance personnel to

reemphasize the importance of ensuring that potential conditions l
prohibited by TS or procedures are properly evaluated and reported.
This will reinforce the practice that evaluations or inspections be conducted by personnel trained and familiar with the specific system or component.

(4) Date When Full Compliance Will Be Achieved The plant is presently in full compliance with Technical Specification 3.7.11.
The above referenced instruction will be provided by Operations and Maintenance Department Management no later than July 31, 1987.
