Investigation on November 2, 1979 (Report No.50-373/79-42)
Areas Investigated:
Investigation was conducted into the licensee's issuance of a Certificate of Compliance (N-5 Data Report) for the Unit 1 feedwater piping subassembly which lacked a required Certified Stress Analysis Report.
Investigation involved a review of procedures, and discussions with the licensee and the licensee's Architect Engineer, Sargent and Lundy, Inc., Chicago, Illinois, The investigation involved a total of sixteen inspector hours by one inspector and one investigator.
Results: The investigation established that on March 14, 1979, the licensee issued the Certificate of Compliance (ASME N-5 Data Report) for the La Salle Unit 1 feedwater piping subassembly and that subsequent to that date initiated an inquiry to verify information contained in a questioned N-5 Data Report, this_ inquiry resulted in the finding that the licensee's piping contractor, Morrison Construction Company had entered formal stress report information from the S&L design drawings as Certified Stress Analysis information in the N-5 form. This resulted in the licensee signing the N-5 form when a p. roper Certified Stress Analsis Report for the Unit 1 feedwater system had not been established by the licensee's Architect Engineer, Sargent and Lundy. No items of noncompliance were identified during this investigation.

REASON FOR INVESTIGATION
>
Investigation was conducted into the licensee's issuance of a Certificate of Compliance (N-5 Data Report) for the Unit 1 feedwater piping subassembly which lacked a required Certified Stress Analysis Report.
SUMMARY OF FACTS Investigation was conducted following the receipt of an anonymous letter which alleged that the licensee had issued a false certification (N-5 Report)
for the Unit 1 feedwater piping subassembly which lacked a required Certified Stress Analysis Report for the subassembly. A previous investigation estab-lished that the licensee bed certified the subassembly, when in fact a Certified Stress Analysis Report had not been established by the licensee's Architect Engineer, Sargent and Lundy.
This investigation addressed the licensee's issuance of the improperly authenticated N-5 Data Report.
Investiga.'.on established that the design information was entered on the N-5 Data Form by the licensee's piping con-tractor, Morrison Construction Company, who transposed formal (incomplete)
stress report information which was listed as Certified Stress Report infor-mation on the questioned N-5 form.
The licensee, after signing the certificate of compliance, initiated an inquiry to verify information entered on the questioned form. This inquiry resulted in the determination by the licensee, that the design information incorporated into the N-5 form was in error, and a Certified Stress Analysis Report for the Unit 1, feedwater piping subassembly was not on file at the Architect Engineer's offices. The licensee's representative subsequently withdrew his signature from the questioned form. No items of noncompliance were identified during this investigation.

Background Information The N-5 Certificate of Compliance for the La Salle County Nuclear Power Station, Unit I feedwater piping subassembly is a document which certifies compliance with ASME Code requirements, as to the design data of field installed Nuclear Power Plant Components and appurtenances. The document itself is a form of certification of design information as to the as-built condition of the subassembly.
This form is executed by the licensee's prime contractor, Morrison Construction Company, Inc., and design information incorporated into the form is provided from drawings issued by the licensee's Architect Engineer, Sargent and Lundy, Inc. The licensee and the ASME code Authorized Nuclear Inspector also sign the form, upon completion of their respective inspections.

Interview with Individual "A" On November 2, 1979, Individual
"A" (CECO) was interviewed and stated that he signed the Certificate of Compliance (N-5 Data Report) for the Unit 1 feedwater piping subassembly on March 14, 1979 at the La Salle County Nuclear Power Station.
Individual "A" related that upon returning to the licensee's corporate of fices he directed his staf f to determine if a Certified Stress Analysis Report for the Unit 1 feedwater piping sub-assembly was on file at the licensee's Architect Engineering firm, Sargent and Lundy's.
"A" related that he was informed by his staff, and that through telephone contacts with Individuals "B" and "C",
it was personally verified by him, that a Certified Stress Analysis Report

was not on file for the Unit I feedwater piping subassembly.
"A" stated in May of 1979 he withdrew his signature from the questioned Certificate of Compliance (N-5 Report).
"A" stated that this constituted an administrative error that did not impact on the safe operation of the feedwater subassembly.

Interview of Individual
"B" Individual "B" (Sargent and Lundy) was interviewed on November 2,1979 and stated that members of his staff advised him sometime in March or April of 1979 that Individual "A" had signed a Certificate of Compliance (N-5 Data Report) certifying the Unit I feedwater system as complete, when as reported in a previous investigation, a Certified Stress Analysis Report had not been completed by the licensee's Architect Engineer, Sargent and Lundy.
Individual "B" attributed this error to a misinter-pretation of Sargent and Lundy design drawings by the licensee's piping contractor, Morrison Construction Company, who transposed formal (incomplete) stress report information from the design drawings and listed it as Certified Stress Report information on the N-5 Data Form.
Individual "B" stated that the N-5 Data Form when signed by Individual "A" appeared to have been properly executed; however, when inquiries from members of Individual "A's" staff were initiated it was learned that the design information incorporated into the N-5 Form regarding the Certified Stress Analysis Report was erroneous in that a Certified Stress Analysis Report as to the "as built" condition of the Unit 1 feedwater system had not been established.
Individual "B" stated he and individual "A" dis-cussed this matter, and following this discussion individual
"A's" signature was withdrawn from the questioned N-5 Data Form.

Interview with Individual "C" Individual C" (Sargent and Lundy) was interviewed on November 2,1979 and stated in March of 1979 a member of Individual
"A's" staff contacted him and requested he verify the design information and stress report information on the Certified Stress Analysis Report (N-5 Form) for the La Salle County Nuclear Power Station, Unit 1 feedwater piping subassembly.
Individual "C" related that after a review of the design drawings it was noted that a Certified Stress Analysis Report as to the "as built" stress analysis of the Unit I feedwater piping subassembly had not been completed by Sargent and Lundy.
Individual "C" related that he informed members of Individual "A's" staff of this discrepancy; and subsequently, in May of 1979, Individual "A" withdrew his signature from the N-5 Data Form.

Management Discussion On November 2, 1979, discussions were held with the individuals previously listed. At this time they were advised of the results of this investigation.
