Reactor Coolant Pressure Boundary Piping a. Observation of Work.and Work Activities The inspector observed piping installation activities for the Reactor Coolant System and the Safety Injection System at Unit 2 to ascertain the compliance with selected requirements of approved procedures; namely, procedures WPP/QCI 400, Rev 8.0, "Piping Installations" and CS-P204, Rev 4.0,

"Field Fabrication and Erection of Nuclear Piping to ASME Section III."

The procedure requirements selected pertained to pipe storage, piping fabrication, installation and inspection. The pipe spools examined were as follows:

-2 System Pipe Spool N Reactor Coolant System S2-1201-ML-042 S2-1201-ML-155 Safety Injection System S2-1204-ML-133 S2-1204-ML-033 S2-1204-ML-044 S2-1204-ML-165 No items of noncompliance or deviations were identifie b. Review of Quality Records The quality records associated with pipe spools no S2-1204-ML-044 and S2-1201-ML-042 were reviewed to ascertain the compliance-of the records with requirements of the licensee's Quality Assurance Program and the ASME B & PV Code.. The records reviewed included:

1) Material Certifications 2) NDE Records 3) Welder Qualifications 4) Chemical and Physical Test Reports 5) Welding Filler Metal Certifications Additionally, the records of six site inspectors performing pipe welding and NDE inspections were examined to the requirements of ANSI N45. No items of noncompliance or deviations were identifie.

Pipe Supports and Restraints Observation of Work and Work Activities The inspector examined nine installed and accepted pipe supports and restraints in the Unit 2 containment, auxiliary building, and safety equipment building. The randomly selected supports and restraints were associated with the containment spray, safety injection, chilled water, volume control, and component coolin systems... The component identification numbers were as follows:

-3 The installation of these supports was compared with requirements included in instructions QCI-414, Rev 4, "Field Fabrication and Installation of Pipe Supports for Project Class A-I and P";

CS-P207, Rev 5, "Pipe Support Field Fabrication and Installation;"

and associated Bechtel installation drawing Eight supports were found to have been installed in accordance with the prescribed drawings and instructions. One support, No. S2-CS-001-H-020 for an eight inch containment spray pipe located in the Unit 2-safety equipment building, was not welded to the pipe wall sleeve flange in accordance with the applicable pipe support assembly drawing (Bechtel drawing S2-CS-001-H-020, Rev 1).

The drawing specifies 5/16 inch fillet welds 2 inches long on 4 inch centerk 'securing two parts (angle steel struts) of the support assembly to th.e sleeve flange. The existing welds were not in accordance with the drawing in that no weld existed on the

.bottom of the top strut and no weld existed on the top of the lower strut. This support had been accepted by Quality Control on January 1.1, 1979. As a result of this finding a nonconformance report (Bechtel NCR P-972, March 8, 1979) was issued by site personne The failure to install the accepted support in accordance with the prescribed drawing is considered to be an item of noncompliance. In addition, the inspector examined mechanical shock suppressors (snubbers) supplied by ITT Grinnell stored in the warehous Several snubbers.were on hold due to bolts attaching the ITT Grinnell rear pivot mount to the Pacific Scientific mechanical unit not being of sufficient length to penetrate the housing.plat This problem was identified to Bechtel site personnel via a Bechtel San Francisco Power Division "Quality Assurance Information Flyer" (Bulletin 10-78, August 29, 1978). Subsequent to this bulletin, Grinnell identified the particular snubbers containing the short bolts and Bechtel placed hold tags on these snubbers pending replacement of the bolts. The inspector found other snubber (not identified by Grinnell) that appeared to have a similar proble The licensee indicated that these snubbers would be inspected to ascertain compliance with thread engagement requirements. This item-is considered unresolved pending results of the licensee's inspection. (50-361/79-08-02)

4. Exit Interview The inspectors met with the licensee representatives (denoted in Paragraph 1) at the conclusion of the inspection on March 9, 1979 and summarized the scope and findings of the inspection as stated in this report. This exit interview was held in conjunction with the NRC Resident Inspector's regularly scheduled weekly meeting.
