Independent Inspection Effort General A general inspection was made inside the primary containment to inspect the activities and work in progress related to the installation of the replacement tube bundle sections and the modifications to the secondary side of the original steam generators. Observations were made of the setting of the "A" generator tube bundle section into the support ring forgings and the initial 3/8" of weld in the girth weld which joins the primary pipe EP-4 to the cold leg nozzle of the "B" generato Observations were also made of the radiography in progress to *
-2-examine the first 3/8 of weld metal in the weld groov The inspector noted that this was the first production weld in the primary coolant piping which was performed using the diametric automatic cold wire tig equipment. It was also noted that the replacement feedwater nozzle sections were completed on all three generators with the exception of the final post weld heat treat (PWIIT) and subsequent xra The PWHT will be performed when the final shell girth weld is made and the stress relieve As of this inspection, none of the girth welds had been fit u No items of noncompliance were identifie Primary Nozzle Welding and Radiography -
An inspection was made of the radiography in progress to examine the first 3/8" of weld metal in the root of the weld joining the EP-4 nozzle to the cold leg side of the "B" steam generato The radiography was performed to the requirements of a licensee approved procedure, RT-A-G-77VS rev. O, which complies with ASME Section III and V, 1974 edition through the winter Addenda of 197 The inspector observed the film placement, radiation source, radiation boundaries, and exposure activitie A visual inspection was made of the weld metal to evaluate weld bead characteristics for cohesion between beads, por<rsity, undercut_s, weld bead roll over or defect indication A visual inspection was made inside the primary head to examine the internal condition of the EB insert weld ring relative to the pull or fused condition of the initial weld layer depositio The inspector noted the presence of QC personnel, licensee surveillance, material and tool control and the general house-keeping of the work are No items of noncompliance were identifie Feedwater Nozzle Radiography An inspection was made of the radiographs for the feedwater nozzles on the "A", "B", and "C" steam generator The nozzles were modified by the replacement of a major part of the nozzle section and incorporating a redesign of the thermal sleeve assembl The radiography was performed and evaluated to the requirements of CBI-RT-9X Rev. 1 which was reviewed for compli-ance to the requirements of ASME Section III and V of the 1974 edition through winter 1976 Addend The inspector noted that*
the radiography was performed with 96 curies of Cobalt 60 through a weld metal thickness of 4.75 inches using Kodak AA film with lead screens in front and bac The film was examined for radiographic quality and to the requirements of the referenced code and the approved procedures for the presence of mechanical, chemical or other blemish~s such as fogging, process marks;:
