Licensee Action on Previous Inspection Findings Not inspecte.

Unresolved Items Unresolved items were not identified during this inspectio.

Independent Inspection Effort The inspector conducted a general inspection of Unit 2 Containment, Auxiliary building, refurbishment building and the CB&I mock-up training

-2-area to observe construction progress and construction activities such as weld joint preparation, welder training, radiation control, movement of new steam generator lower shell into the containment, material handling and control, housekeeping and storag No items of noncompliance or deviations were identifie.

Steam Generator Replacement Observation of Welding and Associated Activities The applicable code as required by "VEPCOs licensee submittal (Steam Generator Project Surry Power Station Unit Nos. 1 and 2, Revision 9)," is ASME Section XI, 1974 Edition with Addenda through the Winter of 197 Based on paragraph IWA-7000 of Section XI, VEPCO is using the original construction code (USAS B31.1.0-1967) for pipe weldin Welder quali-fications and procedure qualifications are being performed in accordance with the latest edition and addenda of ASME section IX in effect at the time of the qualificatio All welding materials were purchased and certified in accordance with Class I requirements of the ASME cod Nondestructive testing is being performed in accordance with ASME code Section III and V, 1974 Edition with Addenda through the Winter of 197 Welder Qualification (1)

The inspector reviewed CB&I Welder qualification certification documentation including radiographs to determine whether welding personnel (manual or automatic equipment) are qualified in accordance with the applicable code requirements as implemented by the CB&I "Nuclear Quality Assurance Manual for ASME Section III Products".

(2)

The inspector reviewed DCC welder qualification certificates and non-destructive test reports to determine whether welding personnel were qualified in accordance with the applicable code requirements as implemented by DCC WP 501 Revision 2 "Qualifica-tion of Welding Procedures and Welders". Welding Identification Identification numbers from the feedwater and lbw level indicator nozzles on steam generator "B" were traced back to weld maps and nondestructive test reports to determine whether these numbers appeared on CB&I weld maps and whether the information was accurate and retrievabl Control of Welding Filler Materials (1)

The inspector observed activities in CB&! electrode issue station in the containment to determine whether conformance with the applicable code and procedural requirements were being implemented in the areas of storage, dispensing, return and disposition of welding electrodes, flux and other material *

(2)

-3-The inspector observed activities in DCC electrode issue stations in the containment and the refurbishment building to determine whether suitable facilities and controls were in place and maintained for storage, dispensing, return and disposition at welding electrodes, flux and other materials, in conformance with the applicable code and procedural requirement Observation of Welding The inspector observed in process weld build-up of weld No 2 on ISO ETA 40004 U2A-EP3 at cut No. 30005-5-U2A EP3; verified that the applicable DCC weld procedure specification was available to the welder and that QC hold points were indicate In addition, the inspector verified that slag removal was performed between layers on machine fabricated weld Repair of Welds The inspector reviewed the below listed documents to determine the adequacy of procedures for the identification and resolution of defects disclosed by NDE methods and implementation weld repai (1)
