NRC INSPECTION REPORT NOS. 50-280/88-16 AND 50-281/88-16 This refers to the Nuclear Regulatory Commission (NRC) inspection conducted by C.H. Bassett on May 2-6, 1988. *The inspection included a review of activities authorized for your Surry facilit At the conclusion of the inspection, the findings were discussed with those members of your staff identified in the enclosed inspection repor *
Areas examined during the inspection are identified in the repor Within these areas, the inspection consisted of selective examinations of procedures and representative records, interviews with personnel, and observation of activities in progres The inspection findings indicate that certain activities appeared to violate NRC requirement The violations described in the enclosed Inspection Report are under consideration for escalated enforcement actio Accordingly, no Nati ce of Violation is being issued at this time, and no response to this letter is require However, please be* advised that the number and characterization of violations described in the enclosed Inspection Report may change as a result of further NRC revie An Enforcement Conference to discuss thes~violations and those violations associated with the overexposure event of May 27, 1988 (Inspection Report Nos. 280, 281/88-25).
Your evaluation of the root causes for violations in this report should consider the root cause analyses performed for the violations noted in the inspection reports documenting the incore detector event (Inspection Report Nos. 50-280, 281/88-10) and the overexposure event (Inspection Report Nos. 280, 281/88-25).
You are requested to discuss the root causes for the programmatic problem in your radiation protection program indicated by these three inspections at the July 6 En~orcement Conferenc It was also noted during the inspection that additional management attention is needed in the area of implementation of improved health phsyics procedures at your Surry facilit Following a procedural review by contractor personnel in 1983, you established a program to revise and upgrade your procedure A corporate Radiation Protection Plan was formally approved to outline functions,
