IEB 80-11 Progra Subsequent to the inspection when this unresolved item was iden-tified, the licensee prepared procedures which required performance of additional inspections in seismic and non-seismic areas of the plant to verify that all walls in the proximity of safety-related equipment had been identified for design re-evaluation, and to document that all class III walls (walls which do not have any category eqiupment in their proximity)

(Open) IE Bulletin 80-11, Masonry Wall Design - Units 1 and 2 Summary of Licensee's Response to IE Bulletin 80-1 Virginia Electric and Power Company ( VEPCO)
submitted to 60 day response to IE Bulletin 80-11 for Surry Units 1 ahd 2 to NRC Region II on July 7, 198 In a letter dated October 24, 1980, VEPCO requested an extension of the deadline for completion of the IEB 80-11 design re-eva 1 uat ion from November 4, 1980 to July 31, 198 VEPCO has submitted three interim reports on the status of their masonry wall re-eva 1 uat ion progra These reports are dated November 3,- 1980, Apri 1 15, 1981, and June 15, 198 Review of Procedures for Accomplishment of IE Bulletin 80-11 Require-ment The inspector examined the following procedures which address the requirements for control of IE Bulletin 80-11 activities:

Procedure BWP-1 addresses the criteria for use in the design re-evaluation of masonry walls and lists the field data for regarding as-built details required for the design re-evaluation. Procedure BWP-4 lists the require-ments for pre pa ration and QC review of masonry wa 11 as-bui 1 t drawings Procedure BWP-8 and BWP-10 were written to reso 1 ve the unreso 1 ved item discussed in paragraph This procedures address the field inspection requirements to identify masonry walls in the proximity of safety-related equipment and the documentation of the field inspection efforts.
e 3 Field Walkdown in Non-Seismic Areas to Identify Masonry Wall The inspector, accompanied by licensee and S&W engineers, walked down the elevation 45 and 47 levels of the service building (non-seismic areas) to verify that the field inspections conducted in these areas were in accordance with the requirements of Procedure BWP-1 The inspector examined wall numbers SB 47-01, SB 47-02, SB 45-3-2 and SB 45-3-The inspector reviewed the records documenting the inspection of these walls and noted that there was no safety-related equipment attached to or in proximity of the wall During the field inspec-tions, the licensee determined that there were 339 masonry walls located in the non seismic areas of the plant. The Field inspection conducted by the licensee and S&W disclosed that none of the 339 walls had any safety-related equipment in their proximit Field Walkdown in Seismic Areas to Verify Accuracy of Masonry Wall As-Built Drawing Subsequent to the December 3-5, 1980 inspection, (See IE Report Numbers 50-280/80-42 and 50-281/80-46) the licensee condu~ted detailed inspec-tions in seismic areas of the plant in accordance with procedure BWP-8 to determine if there were any additional walls in these areas which had not been identified during in-office review of floor plan drawing performed by S&W in their Boston office. As discussed in paragraph 3, this field review identified several additional walls in the proximity of safety-related equipment which required design re-evaluation. The inspector, accompanied by licensee and S&W engineers, examined some of the masonry walls which were identified during these field inspections to verify the accuracy of the as-built drawings which had been prepared for each of the walls. Walls examined were as follows:

Intake Structure Walls (ID Numbers) IS-27-2-1, IS-27-2-2 (2)
Unit 1 Containment Building Walls (ID Numbers) lC 18-4-1 through lC 18-4-14 In review of the as-built drawing for the above walls, the inspector noted discrepancies between field conditions and the details indicated on the as-built drawings. The discrepancies were as follows:

The orientation of the angle supports on the inside faces of walls lC-18-4-1 and lC-18-4-3 are shown on the as-built drawings incor-rectl (2) A note on the as-built drawing indicates that the support angles on the inside face of wall lC-18-4-2 were installed flush with the concret Examination of this face of the wall disclosed that there is a one to two inch gap between the support angles and the concrete wa 11 s.

Paragraph 4.0 of procedure BWP-4 requires the quality control personnel to assure completeness and correctness of the masonry wall as built sketche The as built sketchs for the above walls had been reviewed and approved by Q The failure of QC to assure the correctness of the as-built drawings for walls lc-18-4-1 through lc-18-4-3 in accordance with the requirement of BWP-4 was identified to the licensee as another example of the violation identified in paragraph Inspection of Completed Structural Modification to Masonry Wall In order to avoid the performance of very complex in elastic structural analysis, the licensee has elected to modify some walls which do not meet design requirements when they are analyzed using an elastic analysis, or when wall configurations are to complex to be analyzed using elastic method The design drawing and details to be used to modify the walls were prepared by S&W and transmitted to the site as design change packages. The inspector examined the modifications to wa 11 numbers IS-27-2-1 and IS-27-2-2 and verified that the modi fi-cat ions were performed in accordance with Design Change Package 80-86!,
11Modi fi cation of Masonry Wa 11 s IS-27-2-1 and 2 11 Acceptance criteria reviewed by the inspector in examination of the modification to these walls were drawing numbers 12846.60-CS-24 AA-4A, CS-24AC2, CS-24AD-3, and CS-15C through CS-15 The inspector also examined the precast reinforced concrete panels which were installed in the Unit 1 con-tainment building to replace masonry wall lc-47-4-20, and verified that the new wall was constructed in accordance with the details contained in Design Change 80-86H. Acceptance criteria reviewed by the inspector in examination of installation of the precast wall panels were drawing numbers 12846.60-CS-14N-l and 12846.60-CS-14M-Review of Quality Records Relating to Masonry Wall Modification The inspector examined the quality records relating to the modification of wall numbers IS-27-2-1 and IS-27-2-2, and the replacement of wall number lC-47-4-20. Records examined were as follows:

Concrete inspection records for fabrication of precast concrete panels for wall lC-47-4-2 Records included prepour inspection cards, concrete delivery tickets, concrete placement cards and the concrete test repor (2)
Certified material test reports for nelson studs (headed concrete archors) and structural steel used in fabrication of wall lC-47-4-2 (3)
QC inspection records for structral steel bolting, concrete anchor installation, welding and painting for the installation of precast panels for wall lC-47-4-20 and for the modifications to walls IS-27-2-1 and IS-27-2-2.
