- Attended exit interview_ Exit Interview The inspection scope and findings were summarized on January 9, 1981, with those persorrs indicated in paragraph 1 abov The scope of the inspection was reviewe The licensee was advised. that no violations or deviations wera identified during this inspectio Licensee Action on Previous Inspection Findings (Open) Unresolved Item: 50-280/80-41-01; 50-281/80-45-01 11Administrative Controls Over Inservice Inspection.
(Note:
This item was listed as item 50-280/80-45-01;_ 50-281/80-45-01 when it was ori gi na lly identified due to a typographical error.) This area of concern was reviewed during discussions with QA, QC, and NOE personne The inspector reviewed existing station administrative procedures without finding any mention of inservice inspec~
tion or inservice inspection coordination except for one statement which I
refers to the monitoring of ISI * contractor wor This item was also discussed with the station manager in the context that the inspector is concerned that there does not appear to be a formal mechanism for insuring that the ma.i ntenance repair and rep 1 acement data and operat i ona 1 hi story data are reported in a meaningful manner to an ISI coordinator who can evaluate the data for impact on the station's ten-year ISI progra.
Unresolved Items Unresolved items were not identified during this inspectio.
Independent Inspection During walk-through inspections of the Unit 1 containmen The inspector observed several piping hanger and support installation activitie The work observed included drilling of concrete for installation of anchor bolts; assembling of supports at the installation sites; fit-up and tack welding of assemblies; and welding of support assemblie During these observations the inspector checked to see that proper approved work instructions were at the job sites; procedure steps were being signed as work progressed; and welding filler materials and other materials and tools were being properly controlle No violations or deviations were identifie.
In s e rv i c e rn spec ti on ( Un i t 2 )
The inspector reviewed the inservice inspection report for the 1980 outage of Surry Unit No. This report was compiled by Westinghouse Nuclear Service Division which has the contract for the inservice inspection at Surry..
The report. also inc.luded the preservice. or base.line inspection data for the-new welds in Class 1 and Class 2 systems completed during the steam genera-tor repair activities in Unit Within the areas examined, no violations or deviations were identifie.
Steam Generator Repair - Observation of Work and Work Activities (Unit 1)
Tha inspector observed the follow-ing* work and work activities associated with the steam generator repai Chicago Bridge and. Iron (CBI) welder training activitie Full thick-ness. mockups representing the steam generator girth weld were being used by CBI to train the welders to properly sequence the weld passes in order to minimize-the chanc~ of trapping slag and other impurities in the weld, to reduce th& chance of lack of fusion typ& of defects and to provide a. suitable surface for intermediate level nondestructive examination *
The inspector observed several of the CBI welders* welding on the mockups and also observed the CBI welding supervisors as they evaluated the training wor Fit-up of Steam Generator 11 C 11 Girth We 1 The fit-up of the steam generator 11 C 11 girth we 1 d was inspected for comp 1 i ance with the fit-up requirements of the CBI welding procedures and the steam generator repair specification (The fit-up had been inspected by CBI and the ASME B&PV Code inspector prior to review by this inspector.)
The installation of resistance heaters for the required preheat was also observed during this inspection of the weld fit-u Welding of Steam Generator 11 C 11 Wel The inspector observed welding activities during the deposit of the first ~-inch of weld metal and the preparations for nondestructive examination of the surface of the first
~-inch of we 1 d meta The contra 1 of the preheat temperatures, we 1 ding filler materials, and welding and burning operations were reviewed during this phase of the inspectio Steam Generator Internals -
The inspector observed various welding, handling, and installation activities associated with the proper assembly of the steam drying equipment in the upper sections of steam generators A and There were no violations or deviations noted during this phase of the inspectio..
Stearn Generator Repair - Review oJWelding and NOE Documentation (Unit 1)
The inspector reviewed the CBI documentation of the welding and NOE activi-ties involved with installation welding of new feedwater nozzles, wet layup nozzles, and narrow range 1 evel taps on the three steam generators in Unit The.:_ reviewed. documentation included record drawings, NOE reports andnonconforrnance_report There were no violations or deviations in this area of inspectio.
VEPCO Construction Quality Control Instructions (Unit 1)
The*inspector reviewed the Quality Control Instructions (QCis) which control the inspection and nondestructive, examination of the safety-related work other than tha stea~ generator welds being don~ by CB This other work i nvo*l ves the pi p*i ng we 1 ds associated with the, steam generator repair* as wel 1 as other evolutions involved with IE Bulletins or updating of other safety-related system The QCis reviewed were as follows:
