Dear Shawn Hafen:

On December 31, 2023, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection at Monticello Nuclear Generating Plant. On January 11, 2024, the NRC inspectors discussed the results of this inspection with you and other members of your staff. The results of this inspection are documented in the enclosed report.

No findings or violations of more than minor significance were identified during this inspection.

SUMMARY

The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting an integrated inspection at Monticello Nuclear Generating Plant, in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. Refer to https://www.nrc.gov/reactors/operating/oversight.html for more information.

No findings or violations of more than minor significance were identified.

PLANT STATUS

The unit began the inspection period in Mode 4. On October 6, 2023, the licensee transitioned the unit to Mode 2 and commenced a reactor startup. The unit reached rated thermal power on October 8, 2023. On December 19, 2023, the licensee lowered reactor power to 32 percent of rated thermal power to perform a control rod sequence exchange and clean the water box side of the main condenser. On December 21, 2023, the licensee returned the unit to rated thermal power. The unit remained at or near rated thermal power for the remainder of the inspection period.

INSPECTION SCOPES

Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors performed activities described in IMC 2515, Appendix D, Plant Status, observed risk significant activities, and completed onsite portions of IPs. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

Licensed Operator Requalification Program (IP Section 03.04) (1 Sample)

(1) Biennial Requalification Written Examinations The inspectors evaluated the quality of the licensed operator biennial requalification written examination administered on October 5, 2023.

Annual Requalification Operating Tests The inspectors evaluated the adequacy of the facility licensees annual requalification operating test.

Administration of an Annual Requalification Operating Test The inspectors evaluated the effectiveness of the facility licensee in administering requalification operating tests required by 10 CFR 55.59(a)(2) and that the facility licensee is effectively evaluating their licensed operators for mastery of training objectives.

Requalification Examination Security The inspectors evaluated the ability of the facility licensee to safeguard examination material, such that the examination is not compromised.

Remedial Training and Reexaminations The inspectors evaluated the effectiveness of remedial training conducted by the licensee and reviewed the adequacy of reexaminations for licensed operators who did not pass a required requalification examination.

Operator License Conditions The inspectors evaluated the licensees program for ensuring that licensed operators meet the conditions of their licenses.

Control Room Simulator The inspectors evaluated the adequacy of the facility licensees control room simulator in modeling the actual plant, and for meeting the requirements contained in 10 CFR 55.46.

71152A - Annual Follow-up Problem Identification and Resolution Annual Follow-up of Selected Issues (Section 03.03)

The inspectors reviewed the licensees implementation of its corrective action program related to the following issues:

(1) Housekeeping trends after hydraulic control accumulator left unsecured between SCRAM banks

71152S - Semiannual Trend Problem Identification and Resolution Semiannual Trend Review (Section 03.02)

(1) The inspectors reviewed the licensees corrective action program for potential adverse trends in the use of wireless gauge readers (WGR) throughout the plant that might be indicative of a more significant safety issue. The inspectors observed multiple corrective actions documents related to improper installation of WGR on non-safety significant related systems as well as concerns of event response due to the presence of WGR in the field. The inspectors reviewed all corrective action documents related to WGR and had further discussions with the project owner to confirm WGR have not been improperly installed on safety-related equipment, and actions to correct any adverse conditions were corrected.

No findings or violations of NRC requirements of more than minor safety significance were identified by the inspectors in the course of this review.

INSPECTION RESULTS

Observation: Housekeeping Trends After Hydraulic Control Unit Accumulator Left Unsecured Between SCRAM Banks 71152A While performing plant status walkdowns on November 14, 2023, during the planned replacement of a control rod drive hydraulic control unit accumulator, the inspectors observed the old accumulator left unsecured between the SCRAM banks. The inspectors raised this concern to the work control senior reactor operator. The licensee took prompt action to secure the accumulator and evaluate the risk. This item was entered into their corrective action program (CAP) 501000079045.

The implementing procedure used to control this seismic concern was FPGEN-HSK01, Housekeeping. Specifically, Attachment 72.e states, items weighing 200 lbs. or more SHALL be secured when near sensitive safety related instrumentation. The licensee performed an evaluation that showed the accumulator would not damage the scram banks in the design base earthquake due to 4-inch curbs located on the bottom of the scram banks.

The inspectors reviewed the requirements of the housekeeping procedure and concluded further walkdowns of plant areas were warranted to verify compliance. The following additional observations were made by NRC inspectors.

FPGEN-HSK01 5.1.8 states, Site personnel shall ensure that no objects that could compromise the integrity or effectively reduce the height of a locked high radiation barrier are placed or stored within three feet of the barrier. On November 20, 2023, inspectors observed the effective height of locked high radiation fence located on the turbine deck to be below 6 feet. The licensee entered item into their CAP as 501000079184.

FPGEN-HSK01 5.8.2 states, Large storage container owners shall conduct a quarterly inspection of the interior contents of large storage containers. On November 20, 2023, the inspectors identified during a discussion with the radiation protection manager that quarterly inspections of large storage boxes were not conducted or exempted. The licensee entered this into their CAP as 501000079179.

FPGEN-HSK01 5.1.212 states, Keep tools and equipment away from areas that could affect plant equipment performance and establish barriers if necessary. On November 28, 2023, inspectors identified a ladder improperly stored next to the site protected area fence. The licensee entered item into their CAP as 501000079455.

The inspectors determined that these observations demonstrate a potential lack of adherence to FPGEN-HSK01, Housekeeping. The inspectors did not identify any findings or violations of a more than minor significance associated with the inspection activities.
