On December 31, 2019, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection at H.B. Robinson Steam Electric Plant. On January 22, 2020, the NRC inspectors discussed the results of this inspection with you and other members of your staff. The results of this inspection are documented in the enclosed report.

SUMMARY

The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting an integrated inspection at H.B. Robinson Steam Electric Plant, in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. PLANT STATUS

Unit 2 began the inspection period at rated thermal power and operated at or near rated thermal power for the entire inspection period.

INSPECTION SCOPES

Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors performed plant status activities described in IMC 2515, Appendix D, Plant Status, and conducted routine reviews using IP 71152, Problem Identification and Resolution. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

Requalification Examination Results (IP Section 03.03) (1 Sample)

The licensee completed the annual requalification operating examinations required to be administered to all licensed operators in accordance with Title 10 of the Code of Federal Regulations 55.59(a)(2), Requalification Requirements, of the NRCs Operators Licenses. During the week of October 28, 2019, the inspector performed an in-office review of the overall pass/fail results of the individual operating examinations, the crew simulator operating examinations, and the biennial written examinations in accordance with Inspection Procedure (IP) 71111.11, Licensed Operator Requalification Program. These results were compared to the thresholds established in Section 3.02, Requalification Examination Results, of IP 71111.11.

(1) The inspectors reviewed and evaluated the licensed operator examination failure rates for the requalification annual operating exam and biennial written examination completed on March 15, 2019.

(2) A routine emergency planning scenario on November 19, 2019. The scenario involved a transformer load tap changer fault, a loss of an instrument bus, a RCS leak, and a loss of coolant accident.

(1) The inspectors evaluated the licensees activities related to long-term operation and monitoring of their independent spent fuel storage installation.

INSPECTION RESULTS

Observation: 71152 Semiannual Trend Review 71152 The inspectors evaluated how the licensee's corrective action program (CAP) responded to issues identified during routine plant walkdowns and daily condition report reviews.

Specifically, the inspectors reviewed and assessed recent issues that involved engineering CAP products, both identified by NRC inspectors and site personnel. The inspectors identified the existence of a potentially adverse trend in engineering CAP behaviors and identified examples where NCR's did not include the details required to both adequately assess the issue and complete any actions in a timely manner. This potential adverse trend was also independently identified by the licensee and entered into the CAP as NCR's 2298040 and 2305928.

The inspectors discussed the planned actions with licensee personnel to address the site trend, such as completion of a causal evaluation, communications to reinforce the CAP standards and expectations with engineering personnel, and creation of a CAP oversight plan to review relevant engineering products. The inspectors determined the licensee's actions were proactive and appropriate.
