Areas Inspected:

Reviev of licensee corrective action taken with respect to matters discussed during a telephone conversation between Region III representatives and mcabers of the licensee's staff on April 21, 1978 and confirmed by "Immedic.te Action Letter" dated April 25, 1978.

These matters related to the apparent falsification of guard firearms qualification

records and vital area door check records at Dresden Station.

Results:

Corrective actions requested in the referenced Immediate Action Letter (IAL) have been satisfactorily completed.

Licensee Corrective Action / Region III Immediate Action Letter dated April 25, 1978 On April 17, 1978, Region III was telephonically advised by the licensee that their audit of guard contractor records and interviews with three guards disclosed that firearms qualification records may have been falsified.

On April 19, 1978 licensee representatives met with RIII to discuss the above matters.

Additionally, RIII was advised that a guard sergeant, on being questioned about guard activities, admitted to the Dresden Station Security Administrator

-

that he instructed guards to complete the door check patrol sheet covering vital area doors even though other guard activities pre-cluded their making the required checks.

On April 21, 1978, RIII called the licensee to discuss actions initiated /to be initiated to correct the licensee-identified items of noncompliance.

This discussion was confirmed by RIII in an "Immediate Action Letter" (IAL) issued April 25, 1978.

By letter dated June 2, 1978 (C. Reed to J. Keppler), the licensee advised Region III of corrective actions taken in accordance with the IAL.

This inspection confirmed the licensee's response.

The following identifies each corrective action irem required by the IAL, the licensee's response to that item as not.ed in their

letter of June 2, 1978, and the inspection findings.

IAL Item 1:

Take immediate steps to requalify those guards iden-tified to date who appear to have questionable or falsified firearms certifications.

Completion of certification will be witnessed by CECO.

To be completed by May 5, 1978.

Licensee Response: A requalification program was undertaken to assure that those guards that had been identified as having

i proper qualification..The requalification started on May 3, 1978 and was originally scheduled to be completed by May 5, 2978.

There was, however, some difficulty in requalifying these individuals and additional time was needed.

Mr. L. Bean contacted Mr. J. Donahue (Region III) on May 5, 1978 and informed him of a required extension until May 18, 1978. to complete the requalification, completed by May 18, 1978

Inspection Findings:

Seven guards were identified by.the licensee

as not having been properly firearms qualified.

During requalifi-cation, three of the seven failed to achieve the minimum score and are no longer acting as members of the Dresden guard force.

The requalification was witnessed by a member of the licensee's staff.

The inspectors reviewed records documenting the requalification.

IAL Item 2: Take immediate steps to determine the extent to which

-

additional guard force members are not fully firearms qualified.

If such non-qualified guards are identified, they will be requali-fied and their requalification witnessed by CECO.

To be completed by May 29, 1978.

Licensee Response:

The requalifications of the remaining members

.

of the guard' force were not questioned. We did, however, initiate requalification for the entire guard force on May 23, 1978.

This

,

requalification is being observed by a Commonwealth Edison represen-tative to ensure requalification is properly completed. We expect the requalification of these individuals to be completed by June 16, 1978.

  • Inspection Findings:

All remaining members of the guard force requalified, with one exception.

The latter is currently serving-j l

as s' watchman (unarmed).

Interviews with members of the guard force disclosed that requalifications were witnessed by represen-tatives of the licensee's staff.

Only authorized individuals who have been fully firearms. qualified will perform as guards.

Licensee Response:

All individuals that were identified as having j

falsified firearms qualifications were not permitted to carry'

weapons..These individuals were not used as " guards" but only as

" watchmen" until the requalification was completed.

Inspection Findings: The licensee's response was confirmed through inspector observations during the course of the investigation con-j

I AL Item 4 : With respect to apparent falsification of vital area

" door check" records, take steps to assure that pertinent door checks are being made as required and associated documentation is maintained.

To be completed by April 28, 1978.

Licensee Response:

Corrective action to assure that door checks are being completed was initiated as soon as we discovered the fact that there was falsification of these documents.

On April 13, 1978 the guard force was instructed that f alsification of. door checks,

would not be tolerated and that anyone coatinuing this practice

,

would be subject to dismissal.

To prevent recurrence and assure that door checks are~ completed and documented, Dresden Station has ordered'and installed at all the exterior vital area doors a key punch clock (Detex) with " key

~

stations." These keys are " punched" at the time the door is checked.

.

Inspection Findings: The referenced April 13, 1978 verbal instruc-tion to guards was confirmed through guard Laterviews.

As of June 1,1978, vital area door checks are being verified through the use of the Detex Key Punch Clock in addition to the door check sheet documentation.

.

Exit Meeting At.the conclusion of the inspection, the results were reviewed with'

the Dresden Station Security Administrator. He was advised that satisfactory corrective action had been conpleted with respect to the items noted in the IAL.

In December 1977 I fired a qualification course for the use of a weapon.

In-firing for score (Ind. B) allowed me to use a bench rest even though he was aware that this was not permitted when firing a qualification course.

I know that (Sec. Adnin.) and (Ind. B) had discussed this matter when (Sec. Admin.) saw that bench rests were being used a day, or a few days, before the firing I have descr16ed above.

The targets I fired into using the bench rest were used 'for my qualifying score.

I do not know what happened to the first small bullseye target.

The questionable qualification records, possible impropri-eties in the manner of conducting f.irearms requalification and the apparent falsification of vital area doors checks were reported to us by members of your staff on April 17 and 19, 1978.

Take immediate steps to requalify those. guards identified to date who appear to have questionable or falsified firearms certifications.
