Postexamination Materials

Dear Mr. Clayton:

Revision 12 of NUREG-1021, Operator Licensing Examination Standards for Power Reactors, ES -4.4, Section B.7, provides for the submittal of a n operator licensing postexamination package containing the following:

(1) the graded written examinations (i.e., each applicants original ans wer and examination cover sheets) plus a clean copy of each applicants answer sheet (2) the master written examinations and answer keys, annotated to indicate any changes made while administering and grading the examinations (3) any questions asked by the applicants and the answers given to the applicants during the written examination (ES -4.3)

(4) all examination administration or postexamination review comments made by the facility licensee and the applicants after the written examination and operating tests (ES -4.3)

(5) the seating chart for the written examination (ES -4.3)

(6) documentation (through a cover letter or other correspondence) of facility licensee management or supervisor concurrence [satisfies ES-4.4, Section A.1, third bullet]

(7) the results of any performance a nalysis for the written examination, with recommended substantive changes (8) Form 1.3-1, Examination Security Agreement, with a pre-and post-examination signature by every individual who had detailed knowledge of any part of the operating tests or written examinations before they were administered

With respect to the operator licensing examinations administered in July and August 2024, Pacific Gas and Electric Company (PG&E) confirms that the above items have been provided in electronic format. This completes the postexamination package submittal.

The threshold for determination of questions to be analyzed was a percentage of greater than 50% incorrect responses. We also analyzed other questions based on applicant feedback following the examination. Based on the results of this exam analysis, training needs will be tracked under notification 51178743 task 15.

  1. 33 9/16 total applicants incorrect ( 56%)

Nine chose B. The correct answer is D.

This question asks how to parallel offsite power to a bus supplied by a DG. The correct answer D states to ensure the synchroscope is rotating slowly in the counterclockwise (SLOW) direction. Distractor B states to ensure the synchroscope is rotating slowly in the clockwise (FAST) direction. This would only be true if paralleling a DG to a bus that is supplied by offsite power. No change to the key was made. The question is technically correct as written, with no construction problems.

  1. 53 11/1 6 total applicants incorrect (69 %)

Eleven chose A. The correct answer is B.

This question asks based on instrument air header pressure lowering to 99 psig, how many air compressors will be running. The correct answer B states only compressors 0- 1 through 0- 5 will be running. Distractor A states all air compressors will be running. This would only be true in this configuration if instrument air pressure lowered to 93 psig. No change to the key was made. The question is technically correct as written, with no construction problems.

2024DCPP NRC Exam Administered August2, 2024 SRO Final Written Examination Analysis

Based on the results of this exam analysis, training needs will be tracked under notification 51178743 task 15.

  1. 84 7/10 SRO applicants incorrect (70.0%)

Six chose A, one chose D. The correct answer is B.

This question asks what procedure is used during a loss of offsite power with all EDGs supplying their bus in order to restore power to non-vital 4kV Bus D and if the supply breakers to the bus must be manually opened or automatically opened. The correct answer B reflects the guidance in EOP E- 0.1, Reactor Trip Response step 9b RNO which states to IMPLEMENT OP AP-26, Loss of Offsite power to restore power to non-vital buses. IAW OP AP-26, Loss of Offsite power, the supply breakers to 4 kV Bus D are manually opened prior to restoring power.

Distractor A implements the correct procedure (OP AP-26) but states the supply breakers automatically open, however this is not true since they must be manually opened. Distractor D states to use EOP ECA- 0.3, but this is not correct since this procedure is used to restore power to vital 4kV buses, not non-vital buses. No change to the key was made. The question is technically correct as written, with no construction problems.

  1. 89 6/10 SRO applicants incorrect (60.0%)

Five chose B, one chose C. The correct answer is D.

This question asks what the overload interlock limit is on the manipulator crane and the purpose it serves. The correct answer D is 2180 lbs to prevent damage to the fuel. Distractor B is 1200 lbs which is not correct since this interlock is only active when the gripper is disengaged. Distractor C has the correct load limit of 2180 lbs but states the purpose is to prevent damage to the manipulator crane. This is not true since the manipulator crane is load tested at a higher load than 2180 lbs. No change to the key was made. The question is technically correct as written, with no construction problems.

  1. 100 5/10 SRO applicants incorrect (50.0%)

Four chose C, one chose A. The correct answer is B.

This question asks the priority for procedure implementation during EOP FR -C.2 after all ECCS accumulators have injected, CSF for Core Cooling is Yellow, now a red path exists on RCS integrity. The correct answer is to complete EOP FR -C.2 since exiting now could lead to a red path on core cooling. This fact is reflected in a caution in EOP FR -C.2. Distractor C states to go to EOP FR-P.1 since a red path is a higher priority however this goes against the guidance in the caution in EOP FR-C.2. Distractor A states to stay in EOP FR -C.2 which is correct but the reason to remain in the procedure states because the CSFs are not addressed until a defined procedure exit is reached which is not incorrect for implementing EOP FR-C.2. This reason is only applicable for EOP ECA-0.0, or red or magenta path. EOP FR-C.2 is a yellow path procedure. No change to the key was made. The question is technically correct as written, with no construction problems.

L232 Student Exam Review

8-Confusion if valve if valve failed to the open position or failed to open. Suggest adding a sentence or statement stating the valve is currently closed.

41-Confusion on what the white button is. Suggest giving a symbol or providing a picture to clarify what the white button is.

70- several minutes is vague. Is this during the boration or after the boration? Suggest making it clear that question is asking after the boration.

84-suggest changing supply breakers to aux feeder breakers to be more clear that it is not asking the load side breakers.
