l CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - RX Power 82 % - Unit Load 900 MWe l DESCRIPTION OF OCCURRENCE:

! At 0814 hours0.00942 days <br />0.226 hours <br />0.00135 weeks <br />3.09727e-4 months <br />, December 3, 1982, during routine operation, the Control Room Operator discovered that No. 2B4 Rod Position Indicator (RPI) was l reading 14 steps lower than the bank demand position indicator. This is greater than the maximum of 12 steps allowed by the Technical l Specifications. The RPI channel was declared inoperable and Action

! Statement 3.1.3.2.la was entered.

l l Movement of the affected rod was suspended and preparations were made for conducting a flux map utilizing the moveable in-core detectors.

The RPI channel was subsequently found to be out of calibration.

Following repair of the instrument, the actual control rod position was determined to be within specification.

APPARENT CAUSE OF OCCURRENCE:

Investigation of the problem revealed that the instrument indicator was out of calibration. No other problems were evident, and although previous, Jimilar occurrences had-been noted, none involved Channel 2B4. The problem was therefore attributed to an isolated instance of l drift in the calibration of the indicator.

1 AHALYSIS OF OCCURRENCE:

Control rod position indication is required to be operable in order to verify the Technical Specification limiting conditions for operation involving rod position are met. Specifications on control rod position ensure that acceptable power distribution limits are-maintained, the minimum shutdown margin is provided, and severity of a rod ejection accident is limited. Action statements impose additional restrictions which ensure the bases are met in the event of limited variations from the basic requirements.

l ANALYSIS OF OCCURRENCE: (cont'd)

As noted, motion of the affected rod was suspended and actual rod position was determined to be within specification. The event therefore involved no risk to the health and safety of the public.

The occurrence constituted operation in a degraded mode permitted by a limiting condition for operation and is reportable in accordance with Technical Specification 6.9.1.9b.

Action Statement 3.1.3.2.la requires:

With a maximum of one rod position indicator per bank inoperable, either:

1) Determine the position of the non-indicating rod indirectly by the moveable incore detectors at least once per 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> and immediately after any motion of the non-indicating rod which exceeds 24 steps in one direction since the last determination of the rod's position, or
2) Reduce thermal power to less than 50% of rated thermal power within 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br />.

j . CORRECTIVE ACTION:

The channel indicator was recalibrated and was satisfactorily tested.

! No. 2B4 RPI was declared operable at 1235 hours0.0143 days <br />0.343 hours <br />0.00204 weeks <br />4.699175e-4 months <br />, December 3, 1982, and Action Statement 3.1.3.2.la was terminated. No further action was deemed necessary in view of the nature of the occurrence.
