i Containment Isolation Valve - Inoperable This report was initiated by incident report 82-492.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - RX Power 82 % - Unit Load 900 MWe DESCRIPTION OF OCCURRENCE:

On November 26, 1982, upon attempting to open Nitrogen Supply Header Valve 2NT32, the operator found that the open indication did not light up on the control console.

Upon investigation, an ore,rator reported that the valve indicated closed.

A more detailed inspection revealed that the valve was not fully closed.

Immediately at 1315 hours0.0152 days <br />0.365 hours <br />0.00217 weeks <br />5.003575e-4 months <br />, the valve was declared inoperable, and Action Statement 3.6.3.c was entered.

An operator was dispatched to close the valve 2NT903 in order to isolate the header.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

APPARENT CAUSE OF OCCURRENCE:

The cause of the inoperability of the Nitrogen Supply Header Valve 2NT32 was internal binding.

No recent similar occurrences were noted, and the event was assumed to be of an isolated nature.

ANALYSIS OF OCCURRENCE:

Primary containment integrity ensures that the release of radioactive materials from the containment atmosphere will be restricted to those leakage paths and associated leak rates assumed in the accident analyses.

This restriction, in conjunction with the leakage rate limitation, will limit the site boundary radiation doses to within the limits of 10 CFR Part 100 during accident conditions.

Action Statement 3.6.3.c requires:

With one or more of the isolation valve (s) specified in table 3.6-1 inoperable, maintain at least one isolation valve operable in each affected penetration that is open and isolate each affected penetration within 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br /> by use of at least one closed manual valve or blind flange.

LER-82-144/03L CORRECTIVE ACTION:

At 1326 hours0.0153 days <br />0.368 hours <br />0.00219 weeks <br />5.04543e-4 months <br />, November 26, 1982, an operator closed Valve 2NT993, restoring containment integrity, and Action Statement 3.6.3.c was terminated.

A leak rate test was satisfactorily performed on Valve 2NT903 to verify containment integrity.

It remains tagged shut under administrative control.

Valve 2NT32 is expected to be repaired during the Unit 2 refueling outage.

No supplemental report will be issued.
