100' Elevation Containment Air Lock - Inoperable.

This report was initiated by Incident Report 82-141.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rn Power 100% - Unit Load 1130 MWe.

DESCRIPTION OF OCCURRENCE:

At 1604 hours0.0186 days <br />0.446 hours <br />0.00265 weeks <br />6.10322e-4 months <br />, May 30, 1982, while entering the containment, maintenance personnel were unable to shut the 100' Elevation Containment Air Lock inner door due to mechanical problems.

The individuals exited the 130' Elevation Air Lock, leaving the outer door on the failed air lock closed.

The 100' Elevation Air Lock was declared inoperable, and Technical Specification Action Statement 3.6.1.3.a was entered retroactive to 1604 hours0.0186 days <br />0.446 hours <br />0.00265 weeks <br />6.10322e-4 months <br />.

The operable air lock door was maintained closed in accordance with the action statement.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The air lock inner door failed to close due to the latch linkage being out of adjustment.

The change in adjustment resulted from normal wear-in of the latch rod and roller assemblies which were replaced during maintenance on May 22, 1982.

ANALYSIS OF OCCURRENCE:

This occurrence resulted in operation in a degraded mode as permitted by a limiting condition for operation, and was reportable in accordance with Technical Specification 6.9.1.9.b.

Failure of one air lock door did not, by itself, result in any degradation of the containment boundary.

The operable door was maintained closed, as noted, and provided containment integrity.

Consequently, no risk to the health or safety of the general public was involved.

LER 82-047/03L.Action Statement 3.6.1.3.a requires:

With one containment air lock door inoperable, maintain at least the operable air lock door closed, and restore the inoperable air lock door to operable status within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />, or be in at least hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in cold shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

CCRRECTIVE ACTION:

The latch linkage was properly adjusted, and the air lock was satisfactorily tested.

At 0150 hours0.00174 days <br />0.0417 hours <br />2.480159e-4 weeks <br />5.7075e-5 months <br />, May 31, 1982, the 100' Elevation Containment Air Lock was declared operable, and Action Statement 3.6.1.3.a was terminated.
