No. 21 Overtemperature Delta Temperature Channel : -

Setpoint Out of Specification.

This report was initiated by Incident Report 82-1:!7.

CONDITIONS PRIOR TO OCCURRENCE: '

Mode 1 - Rx Power 98 % - Unit Load 1110 MWe.

DESCRIPTION OF OCCURRENCE:

At 1100 hours0.0127 days <br />0.306 hours <br />0.00182 weeks <br />4.1855e-4 months <br />, May 18, 1982, during routine channel functional. .-

surveillance testing, it was discovered'thht the No. 21 Over-temperature Delta Temperature (OTAT) Channel trip actuated higher than allowed by Technical Specification 2.2.1 rable 2.2-1.

Action Statement 2.2.1 was entered at that. time; f.he channel wzs accordingly declared inoperable, and Limiting Coddition for -

Operation 3.3.1 Action 6 was entered. The OTAT trip function

was still operable, however, due to operable redundant channels.

DESIGNATION OF APPARENT CAUSE OF OCCURRENdE:

The trip setpoint was outside allowable. limits 'due to contacts on the lead / lag-derivative switch in the lead / Jag amplifier failing to complete the trip circuit. In the opinion'of experienced Instrument and Control personnel, this problem was due to dust -

on the switch contacts, and was of an isolated nature.

i ANALYSIS OF OCCURRENCE:

This cccurrence involves reactor pro,tection system settings which i are found to be less conservative than those established by the- '

l Specification, but which do not prevent.the fulfillment of system L -

functional requirements. It is therefore, reportable in accordance ,

l with Technical Specification 6.9.1.9.2.

Due to system redundancy, the trip function was still operable, and l consequently, no risk to the health or safoty of the general public.

f'f ANALYSIS OF OCCURRENCE: (continued) 1; Action Statement 2.2.1 requires:

With a reactor trip system setpoint less conservative r than the value allowed by Table 2.2-1, declare the channel inoperable and apply the applicable action statement required by Technical Specification 3.3.1 until the channel ic restored to operable status with its trip setpoint consistent with Table 2.2-1.

Limiting Condition for Operation 3.3.1 Action 6 requires:

With the number of operable channels one less than the total number of channels, power operation may continue provided the inoperable channel is placed in the tripped condition within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />.

CORRECTIVE ACTION:

The channel was left in the tripped condition in compliance with the action statement. The switch was cycled to clean the contacts, and the channel was returned to service; no further repair was deemed necessary to prevent recurrence, in view of the nature of the failure. No. 21 OTAT channel was satisfactorily tested and was declared operable at 1402 hours0.0162 days <br />0.389 hours <br />0.00232 weeks <br />5.33461e-4 months <br />, May 18, 1982. Limiting

- Condition for Operation 3.3.1 Action 6 and Action Statement 2.2.1 were terminated at that time.
