Containment Service hater Leak - No. 22 Containment Fan Coil Unit.

This report was initiated by Incident Report 82-128.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 100% - Unit Load 1150 MWe.

DESCRIPTION OF OCCURRENCE:

At 0715 hours0.00828 days <br />0.199 hours <br />0.00118 weeks <br />2.720575e-4 months <br />, May 19, 1982, during maintenance on No. 22 Containment Fan Coil Unit (CFCU), a 1.5 GPM leak was discovered inside containment, on the service water piping to the unit cooling coils.

The leak was immediately isolated, and at 0720 hours0.00833 days <br />0.2 hours <br />0.00119 weeks <br />2.7396e-4 months <br />, the NRC was notified by telephone, in accordance with the Technical Specifications.

The incident was subsequently overlooked until l

June 3, 1982, due to inadequate implementation of Administrative Procedure AP-6.

This occurrence constituted abnormal degradation of the containment boundary in accordance with Technical Specification 6.9.1.8.c.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The leakage resulted from an eroded blank flange in the service water piping to No. 22 CFCU.

ANALYSIS OF OCCURRENCE:

l NRC IE Bulletin 80-24 requires:

That a service water leak inside containment be considered as a degradation of the containment boundary in accordance with Technical Specification 6.9.1.8.c.

CORRECTIVE ACTION

The blank flange was properly replaced and the leakage was stopped.

At 0330 hours0.00382 days <br />0.0917 hours <br />5.456349e-4 weeks <br />1.25565e-4 months <br />, May 22, 1982, maintenance on No. 22 CFCU was completed, and the unit was declared operable.

The personnel involved were counseled concerning Administrative Procedure AP-6 and NRC IE Bulletin 80-24 by the Senior Shift Supervisor.
