Equipment Description MCC 1A Other Advee Conditions 1I. Have you looked for and found no other seismic conditions that could Y0R NO UO adversely affect the safety functions of the equipment?

InternalsInspection has been performed and the following condition has been documented in CR 719207:

While performingthe NTTF 2.3 Seismic Walkdown for Farley Unit 1 for 1A MCC, there were two Instances identified where selsmically qualifiedAgastat Relays were Installed with only three of the four requiredmounting screws. This condition haspreviously been evaluated for the 2A MCC, reference TE 640206 for the evaluation, and it has been determinedthat the current mounting configuration does not adversely Impact the functionalityof any of the relays In any of its operatingmodes (before, during, and after a DBE). The U2 evaluation has been reviewed and is bounding for the conditions found on UI 1A MCC. Therefore, the condition does not bring Into question the operabilityor functionality of the relays.

Although the relays remain functionaland operable, the relays should be reinstalledwith the intended mountingconfiguration of four screws at the earliest opportunityto return margin to the relays.

The relays associatedbreakercubicle locationsand affected loads are documented below:

BKR FAJ4R - Agastat Relays for BKR FAJ5 (1C CTMT POST ACCIDENT AIR MIXING FAN)

BKR FAJ4L - Agastat Relays for BKR FAIS (ID CTMT POST ACCIDENT AIR MIXING FAN)

Based on the evaluation for the as-found conditionof the Agastat relays being acceptable, thiscondition does not representan adverse seismic condition.

Comments (Addilional pages may be added as necessary)

External insection performed 8/29/12.

Second walidown performedon 10/16/13 to Inspectcabinet Internals. No adverse seismicconditions were noted during the final walkdown.

11. Have you looked for and found no other seismic conditions that could YS NO UD adversely affect the safety functions of the equipment?

All cables have adequate flex. The leftmost Termnal block in cabinet FNB1 is missing one of the lower mounting screws. This is judged to not be a seismic concern due to the minimal weight of the block (<10 Ibs). Three of four screws areIntact and in good condition. CR 722630 has been written to document this condition and request replacement.

Comments(Additional pages may be added as necessary)

Externalinspectionperformed on 8/29/12.

Anchorage modification has been performed to resolve IPEEEvulnerability. Cabinethas been bolted to back wall to enhance anchorage.

Second walkdown to Inspect internalson 10/18/13 found no adverse seismic conditions.

Secondary walkdown and SWC performed 10/18/13 has been retyped for clarity.
