Several enhancements have been made to the anchorage incudlag:shlmdrrnga gap between the panel and groutpad; additIonal 3S expansion anchorson the back (North)side of the cabinet; anda 114' plate welded to the frame which Is anchoredby (2)3/8" expansionanchorsnear the font (South) side of the panel. Therefore, this condition Is judged by the SWEs to not be an Immedat concern. However, repairsshould be made to the existinggrout nearthe cast-in-placeanchors.

2.Internals Inspection and anchorageverification was performed to the extent possible. All front and rearpanels were removed and Inspected; however, a second, internalpanel was not removed In the rearof the swltchgear. A review of the associatedSEWS packages for 4.16 kV Switchgear shows that QIR15AS04 (WJ) Is bounded by SEWS package for Q1R15A505 (1K), which is the governingpackage for all 4.16 kV Swltchgear. As statedin the SEWS package for Q1R15A503 (1H), additionalInspections were performed to provide a level of confidence that all4.16 kV Switchgearwere installedproperly. An additionalconservative anchoragecalculation(S0-93 8685-001) was performed for six othersimilarswltchgearusing GIP criteriato prove all units had acceplableanchorage.In total, eight switchgearhad visual verification of anchrage which provides a high level of confidence that all swltchgearwere Installedproperly.

3. SWC performed and signed originally on 10/11/13. Revised and re-signed to provide additional Information on anchorageverification.
