Equipment ID No. 0SH22L502 Equip. Class, 20 Equipment Description DIESEL LOCAL RELAY PANEL 1-2A Interaction Effects

7. Are soft targets free from impact by nearby equipment or structures? Yo NC UD N/Ar-There is a 1/2" gap between panel and QSH21E525.A. Panel E525-A is very long in that directionand can expect to have negligible deflections. The Diesel Local Relay Panelwill also have small deflections as it is restrainedby four anchorbolts and rigid conduits at the top of the panel. Therefore, this is seismicallyadequate.
8. Are overhead equipment, distribution systems, ceiling tiles and lighting, Yo NO U177 N/A' and masonry block walls not likely to collapse onto the equipment?
9. Do attached lines have adequate flexibility to avoid damage? YZ NEI UO N/AC

Other Adverse Conditions 11, Have you looked for and found no other seismic conditions that could Y0Z NO UI]

adversely affect the safety functions of the equipment?

All IPEEEconcerns were resolved. A new anchorbolt was added to the outside of the panel to resolve the missing anchorbolt found in the IPEEE walkdowns. Also, overhead lights have been adequately restrained, therefore this configurations is judged to be free of seismic interaction.

Seismic Walkdown Checklist (SWC)

Equipment ID No. QSR42B5268 Equip. Class' 16 Equipment Description DIESEL WATER BUILDING BATTERY CHARGER NO. 2 (STANDBY)

Location: Bldg. SWIS Floor El. 188 Room, Area 72D Manufacturer, Model, Etc. (optional but recommended)

Instructions for Completing Checklist This checklist may be used to document the results of the Seismic Walkdown of an item of equipment on the SWEL. The space below each of the following questions may be used to record the results of judgments and findings. Additional space is provided at the end of this checklist for documenting other conunents.

Anchora2e

1. Is the anchorage configuration verification required (i.e., is the item one YZ No of the 50% of SWEL items requiring such verification)?
2. Is the anchorage free of bent, broken, missing or loose hardware? Y0 NO UC N/AC
3. Is the anchorage free of corrosion that is more than mild surface YZ NO UC- N/AO oxidation?
4. Is the anchorage free of visible cracks in the concrete near the anchors? YO N- UC N/A[C]
5. Is the anchorage configuration consistent with plant documentation? YN NO UC N/AC (Note: This question only applies if the item is one of the 50% for which an anchorage configuration verification is required.)

DrawingU-732799 Version 1.0

6. Based on the above anchorage evaluations, is the anchorage free of YN N" UO potentially adverse seismic conditions?

IEnter the equipment class name from Appendix B: Classes of Equipment.

.Interaction Effects

7. Are soft targets free from impact by nearby equipment or structures? YN NE UI- N/AO
8. Are overhead equipment, distribution systems, ceiling tiles and lighting, YZ NQl UQ N/AQ and masonry block walls not likely to collapse onto the equipment?

Overheadlighting is attachedby conduit and hooks which does not meet the requirementfor a positive end connection. However decking above prevents hooks from coming disengaged. This is judged to not collapseon any equipment.

9. Do attached lines have adequate flexibility to avoid damage? YO NO UE N/AF
10. Based on the above seismic interaction evaluations, is equipment free YZ NO UO of potentially adverse seismic interaction effects?

Other Adverse Conditions

11. Have you looked for and found no other seismic conditions that could Y0 NEI UE" adversely affect the safety functions of the equipment?
