fromtheworkrequest.Thesecondfailedbarrierinvolvedinconsistent

formatting

oftheinstrument

datasheetforMS-SUM-608.Typically,

thedatasheetforthistypeofsummershowsthe5pointchecksforbothchannel1and2gaintestsonthefrontpage.However,thedatasheetforMS-SUM-608

showsonlythe5pointchecksforchannel1gaintestonthefrontpage,andchannel2checksareshownonanaddenda.Duetothisinconsistency

intheformatofthedatasheet,andthelackofaquestioning

attitudebystationI&Ctechnicians,

thechannel2checkswereinconsistently

performed.

Thechannel2checksforMS-SUM-608havebeenshownontheaddendatothedatasheetsince1983.Theonlydocumented

channel2dataforMS-SUM-608

wasrecordedin1984.MS-SUM-608

wasnotusedasasourcefortotalcoreflowinthesubjectsurveillance

until1994whenitwasaddedtotheprocedure.

Sincethattimetheplanthasnotbeenoperatedinsingleloopmode.Therefore,
