area(HHRA)wascausedbyhumanerroronthepartofahealthphysicstechnician.

Onecondition

thatcontributed

totheeventwasthefailuretoperformatimelyindependent

verification

ofthedoor'slockedstatus.Asecondcontributing

condition

wastheuseofachaintosecurethegate,ratherthanpermanently

installed

padeyes.RREANTAKENANDRTSAHIEVEDUponidentification

oftheeventahealthphysicstechnician

responded,

visuallyverifiedtheareatobeclearofpersonnel

andimmediately

lockedthegate.Awalk-down

wasperformed

toverifythelockedstatusofallothersimilarareas.Nootherdoorswerefoundtobeimproperly

secured.Anadditional

walk-down

wasperformed

byHealthPhysicstoidentifyanyotherHHRAorhighradiation

areasthatweresecuredbymeansofchainandpadlocks.

Oneadditional

door(accesshatchtothewetwell)wasfoundtobesecuredinthismanner.SupplySystemstaffdetermined

thatthishatchdidnotrequireapadeyeasitisboltedandsecuredwithaheavychainandpadlock.
