Title | Licensee-Identified Violation |
Description | TS LCO 3.6.1.3, Primary Containment Isolation Valves, required that each Primary Containment Isolation Valve (PCIV) be operable in Modes 1, 2, and 3, and when the associated instrumentation was required to be operable according to TS LCO 3.3.6.1, Primary Containment Isolation Instrumentation. For one or more inoperable excess flow check valves (EFCVs), TS 3.6.1.3 required the affected flow path to be isolated within 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />, or be in Mode 3 in 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> and Mode 4 in 36 hours4.166667e-4 days <br />0.01 hours <br />5.952381e-5 weeks <br />1.3698e-5 months <br />. However, during TS required surveillance testing during the U3C14 RFO, the licensee discovered that five of 15 EFCVs failed to isolate. Based on the existence of multiple failures the licensee concluded that one or more EFCV was inoperable during fuel Cycle 14 operation. This TS violation was entered into the licensees CAP as PER 222850. The finding was determined to be of very low safety significance (Green) because it did not represent an actual open pathway in the physical integrity of primary containment and did not contribute to the increased potential of an reactor coolant system instrument line break. |
