SUBJECT:

- TMI-1 OTSG'S WESTINGHOUSE STYLE ROLLED PLUGS On Juns 25, 1984, it was discovered that OTSG tube number A-135-72 was not plugged in the upper tube sheet (plug later found in different tube), although it was required to be plugged and supposedly plugged in June 1983 with a Westinghouse Style Mechanical Rolled plug. A complete photographic verifica-tion of all plugged tubes containing this type plug was performed by the licensee following this discrepancy. This verification revealed that five additional Westinghouse plugs (three plugs in the "A" generator's lower tube sheet and two in the "B" generator's lower tube sheet) were missing. Discuss-ions with the licensee indicate a test program will be developed by GPUN Techn.ical Functions, in conjunction with Westinghouse, to ensure that the remaining plugs are in tact. In addition, a safety analysis is being prepared by GPUN Technical Functions, in conjunction with B&W, to confirm the prelimi-nary evaluation that the presence of the five missing roll.ed plugs in the bottom of the reactor vessel will not impair the thermal hydraulic performance of the reactor.

This forwards the proposed Task Interface Agreement (TIA) (Enclosure 1) requesting NRR take lead responsibility on review pf the TMI-1 OTSG plug problem and evaluate GPUN's analyses and proposed corrective actions. This review will need to be completed prior to the TMI start-up, estimated to be in the fall of 1984.

ACTION PLAN:

NRR: (1) Assume lead responsibility for resolution of loose and missing Westinghouse rolled plugs identified by licensee in letter dated July 3, 1984 (Hukill, GPUM to Stolz, NRR) ,

-(2) Evaluate technical and safety aspects of loose and missing plugs identified at TMI-1. Evaluation should address licensee's inspection program and findings, lic'ensee's plans for operation, and repair

. program. -

(3) Review the results of QPUN test program and safety analysis addressing this subject.

(4) Designate Lead Project Manager to assign TACs.and coordinate corre-spondence, meetings, and reports, if necessary. Prepare final report .
