APPENDIX A NOTICE OF VIOLATION Connecticut Yankee Atomic Power Company Docket No. 50-213 Haddam Neck Plant License No. DPR-61 As a result of the inspection conducted on January 6-9, 1987, and in accordance with the " General Statement of Policy for NRC Enforcement Actions," 10 CFR 2, Appendix C (Enforcement Policy), the following violation was identified:

10 CFR 50, Appendix B, Criterion V, states in part that activities affecting quality shall be prescribed by documented instructions, procedures, or drawings and shall be accomplished in accordance with these instructions, procedures, and drawings; Criterion II further states that the quality assurance program shall provide control over activities affecting quality.

Contrary to the above, as of January 9, 1987, the control provided by procedure ACP 1.2-6.4, Temporary Procedure Change, Rev.18 over changes and substitution of test equipment was not adequate in that it permitted test personnel to change, modify, and/or substitute test methods without any higher supervisory or technical review, and without documenting such changes, modification or substitutions in the test procedures.

This is a severity level IV violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Connecticut Yankee Atomic Power Company, is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or expla-nation in . reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
