ApPENDfX A NOTICE OF VIOLATION Long Island Lighting Company Docket No. 50-322 Shoreham Nuclear Power Station License No. NPF-36 l'

As a result of the inspection conducted on July 13-17, 1987, and in accordance with:the NRC Enforcement Policy (10 CFR 2, Appendix C), the following

violations were identified:

A. 10 CFR 50 Appendix B, Criterion V states in part, that " Activities affecting quality shall be prescribed by documented instructions... and shall be accomplished in accordance with these instructions..."

The licensee's Engineering and Design Coordination Report (E&DCR) No.

F-73200 approved on July 30, 1979 requires in part:

"This application (Raychem Nuclear Grade End Caps) shall not be used in primary (containment) for class. IE systems.

Contrary to the above, on or before July 14, 1987 Raychem Nuclear Grade l End Caps were applied.to the splices on the Solenoid Operated Valves (SOV's) electrical leads in Terminal Box T46*50V-39A located at Elevation 140' and azimuth angle 180 of the drywell, a part of the primary containment.

This is a Severity Level V Violation. (Supplement I)

B. ' Technical Specification 4.0.4 requires in part that " entry into an Operational Condition shall not be made unless the surveillance requirements associated with the Limiting Condition for Operation have been performed."

Contrary to the above on May 22, 1987 entry into Operational Condition 2 was made without verifying that the Boron-10 enrichment was 85 atom percent by analyzing a sample taken from the standby liquid control tank as required by Technical Specification 4.1.5.e.l.

This is a Severity Level IV Violation. (Supplement I)

Pursuant-to the provisions of 10 CFR 2.201, Long Island Lighting Company is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
