Long Island Lighting Company Docket No. 50-322 This refers to the inspection conducted by representatives of the Region i*-

I (Philadelphia) office at the Shoreham Nuclear Power Station, Shoreham, New York, of activities authorized by NRC Construction Permit CPPR-95.

apparent items of noncompliance were identified.

I.

I 10 CFR 50, Appendix B, Criterion IX requires that welding be accom-plished using qualified procedures in accordance with' applicable

codes and gecifications.

The Shoreham Nuclear Power Station FSAR, Chapter 17, Paragraph

.j 17.1.9 B, states, in part: "The S&W Quality Assurance Program...

{

fsI provides assurance that approved special process procedures are used... in accordance with specified codes...(and) requires that special process controls... be imposed by specification..."

The S&W specification SH1-327, Radial Beam and Pipe Break Restraint Installation, states, in part: " all welding shall conform to the Structural Welding Code AWS Dl.1."

The Structural Welding Data Sheet for the pipe break restraints FWR 1, 2, and 15 directs that the welding is to be performed in accordance with Welding Procedure Technique Sheet W70H(0), which, in turn, i

states that, "This procedure is prequalified in accordance with AWS D1.1-72."

The AWS Structural Welding Code D1.1, paragraph 2.9 limits the weld joint angle for single-bevel-groove welds to 45' minimum.

,'j Contrary to the above, on August 9,1978, the single-bevel-groove weld joint angles for pipe break restraints FWR 1, 2, and 15 were

This is an infraction.

10 CFR 50, Appendix B, Criterion V, states, in part:

" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings... and shall be accomplished in accordance with these instructions, procedures, or drawings."

The Shoreham Nuclear Power Station FSAR, Chapter 17, paragraph 17.1.5B., states, in part:

"The S&W Quality Assurance Program...

requires that quality activities are based on engineering specif-ications... (and) these documents indicate... the applicable codes..."

The S&W engineering specification SH1-24, Specification for Shop Fabricated Piping, requires that code class 3 piping systems be designed, fabricated, and inspected in accordance with the ASME III

Pipe spools 1821-SLP-203-1 and 1821-SLB-211-3-1 are 10-inches diameter standard weight pipe, Class 3.

For these, paragraph i

ND3661.1 of the ASME III Code requires slip-on flange front weld size (inside diameter) to be 1/4 inch and back weld size (outside diameter) to be 0.511 inch.

Contrary to the above, on August 9,1978, the front weld (inside diameter) size on pipe spool slip-on flange 1821-SLP-211-3-1 was less than 1/4 inch. The back weld (outside diameter) size on pipe spool slip-on flanges 1821-SLP-211-3-1 and 1821-SLP-203-3-1 was less than 3/8 inch.

This item is an infraction.
