DPR-22 Failure of tb.1 TIP Ball Valve to Isolate A condition occurred at the Monticello Nuclear Generating Plant which we are l reporting to your of fice in accordance wi th Section 6.6.C.1 of the Technical Speci fications of Provisional Operating License DPR-22.

l l On February 2,1973, during testing of the T raversing in-Core Probe (TIP) System, l the fb.1 TIP ball valve failed to isolate when the probe was retracted back into j the chamber shield. The ball valve was replaced with a new valve and proper operation of the new valve veri fied, inspection of the replaced valve revealed the presence inside the valve and on the valve seat of material used to lubricate the TIP cable. Also, i t was found that the valve rotor spring tension, which is adjustable, was less than the spring l tension of the new valve. The cable lubricating material caused the valve to l bind and insuf ficient spring tension existed to rotate the valve to the closed l position. I t should be noted that if the TIP System had been requi red to isolate, isolation could have been accomplished by actuating the shear valve upstream of the ball valve.

Procedures have been issued to require a verification of proper isolation of the three T IP ball valves daily.
