34,225 uci/sec j DESCRIPTION At 1125 on Harch 10, 1975, an operator was dispatched to the l OF OCCURRENCE:

boiler house to changa over from the "B" stack gas sampic pump to the "A" pump. Since it had been reported that the "A" p. imp was losing oil, the intent was to opcrate this pcmp long cnough for maintenance personnel to troubicshoot.

Noting that the "A" pump was cxtrcmcly noisy upon starting, the operator advised the shift foreman of this condition. The shift forccena in turn, dispatched an assisting shift forc=an to evaluate the prob 1cm. The "A" stack gas sample pump subscquently tripped off while the assisting shift foreman

He then resct the thermal overload (common to both stack gas sampic pumps) and switched back to the "A" pump. The "A" stack gas sample pump started at this time. t-lhile the "A" pump operatcl it was noted that flow was low, (about 1 cfm) and that vacuum was highcr than

. normal. Consequently, the assisting shift forcman reset the thctmal overinad (as an additional precaution) and started the "B" stack gas sampic pump. The total amount of timo during which ncither stack gas sampic pump was operating was icss than one (1) minutc.

The cause of the failure of the "D" stack gas sampic purrp to start following the trip of the A" pump 'is con ~idered to be circuit design. The cxisting circuit provides just onc thermal overload for both pumps. Consequently, a trip of one pump on thormal overload requires the manual rcsetting of the thermal overload beforc the other stack gas samplo pump can be started.

The cause of the trip of the "A" stack gas sample pump is currently under investigation.
