On March 5,2013, the U.S. Nuclear Regulatory Commission (NRC) completed an examinationat Indian Point, Unit 3. The enclosed report documents the examination findings, which werediscussed on March 6,2013, with Mr. Frank Inzirillo and Mr. Steve Davis of your staff.The examination included the evaluation of two applicants for reactor operator licenses, and fiveapplicants for instant senior operator licenses. The written and operating examinations weredeveloped using NUREG-1021, "Operator Licensing Examination Standards for PowerReactors," Revision 9, Supplement 1. The license examiners determined that all applicantssatisfied the requirements of Title 10 of the Code of Federal Regulations (10 CFR) Part 55, andthe appropriate licenses were issued on March 6, 2013.No findings were identified during this examination.In accordance with 10 CFR 2.390 of the NRC's "Rules of Practice," a copy of this letter and itsenclosure will be available electronically for public inspection in the NRC Public DocumentRoom or from the Publicly Available Records (PARS) component of NRC's document systemAgencywide Documents Access and Management System (ADAMS). Scope.2b.a.The examiners reviewed all license applications submitted by the licensee to ensure theapplications reflected that each applicant satisfied relevant license eligibilityrequirements. The applications were submitted on NRC Form 398, "PersonalQualification Statement," and NRC Form 396, "Certification of Medical Examination byFacility Licensee." The examiners also audited all of the license applications in detail toconfirm that they accurately reflected the subject applicant's qualifications. This auditfocused on the applicant's experience and on{he-job training, including controlmanipulations that provided significant reactivity changes.FindinqsNo findings were identified.Operator Knowledqe and PerformanceExamination ScopeOn February 20,2013, the licensee proctored the administration of the writtenexaminations to all seven applicants. The licensee staff graded the writtenexaminations, analyzed the results, and presented their analysis to the NRC onFebruary 27,2013.The NRC examination team administered the various portions of the operatingexamination to all seven applicants on February 11-15,2013. The two applicants forreactor operator licenses participated in three dynamic simulator scenarios, in a controlroom and facilities walkthrough test consisting of 11 system tasks, and an administrativetest consisting of four administrative tasks. The five applicants seeking an instant senioroperator license participated in either two or three dynamic simulator scenarios, acontrol room and facilities walkthrough test consisting of ten system tasks, and anadministrative test consisting of five administrative tasks.FindinqsAll applicants passed all parts of the operating test. For the written examinations, thetwo reactor operator applicants both received a score of 84 percent. The senioroperator applicants' average score was 85.8 percent and ranged from 80 to 91 percent.The overallwritten examination average was 85.28 percent.b.Enclosure

.32The NRC examiners completed the final grading of the written examination onMarch 5, 2013, and conducted a review of each missed question to determine theaccuracy and validity of the examination questions.Chapter ES-403 and Form ES-403-1 of NUREG 1021 require the licensee to analyzethe validity of any written examination questions that were missed by half or more of theapplicants. The licensee conducted this performance analysis for all questions that metthese criteria and submitted the analysis to the chief examiner. No post examcomments were submitted.The text of the examination questions and the licensee's examination analysis may beaccessed in the ADAMS system under the accession numbers noted in Attachment 1. Inaccordance with current NRC policy the release of this written examination will be delayedfor 2 years.Initial Licensing Examination DevelopmentExamination ScopeThe facility licensee developed the examinations in accordance with NUREG-1021,Revision 9, Supplement 1. All licensee facility training and operations staff involved inexamination preparation and validation were listed on a security agreement. The facilitylicensee submifted both the written and operating examination outlines onNovember 13,2012. The chief examiner reviewed the outlines against therequirements of NUREG-1021, and provided comments to the licensee. The facilitylicensee submitted the draft examination package on December 13, 2012. The chiefexaminer reviewed the draft examination package against the requirements ofNUREG-1021, and provided comments to the licensee. The NRC conducted an onsitevalidation of the operating examinations and provided further comments during theweek of January 12,2013. The licensee satisfactorily completed comment resolution onFebruary 19,2013.FindinssThe NRC approved the initial examination outline and advised the licensee to proceedwith the operating examination development.The examiners determined that the written and operating examinations initiallysubmitted by the licensee were within the range of acceptability expected for a proposedexamination.No findings were identified.a.Enclosure

Plans for simulator security and applicant controlwere reviewed and discussed with licensee personnel.FindinqsDuring the validation week, a potential compromise occurred when an alarm printer wasreloaded with paper by an ILO instructor and an alarm related to one of the examscenarios printed out. The facility immediately notified the chief examingr, and theaffected scenario was modified.
