Dear Mr. Spessard:

This is in response to your Notice of Violation and Inspection Report Number 50-461/81-25. Illinois Power Company's responses to the two violations identified are as followsi

, 1. Handling, Storage, and Shipping The Notice of Violation states in part:

" Contrary to the above, physical and environmental protection from nearby construction activity was not provided for five installed Class IE' Electrical Cabinets and numerous pre-cut, coiled, temporarily stored Class lE Electrical Cables."

A. Corrective action taken and the results achieved:

The construction materials and debris on and around the referenced cabinets were removed and protective coverings were restored where required.

Precut, coiled. cables were moved to flagged staging areas which' segregated the cables from '

ongoing construction activity.

B. Corrective action taken to avoid further noncompliance:

'Previously, maintenance of physical and environmental protection of equipment was accomplished under the direction of various discipline Superintendents when notified by the Storage and Maintenance Engineer. This method has proven to be cumbersome and slow to react to requests / notification by the Storage and Maintenance Engineer. Therefore, special crews-have been assigned to maintain equipment physical-and environmental protection requirements at the

~ direction of the Storage and Maintenance Engineer.

Spessard Page 2 January 8, 1982 effectiveness of maintaining equipment storage requirements and in correcting any deficiencies identified by the Storage and Maintenance or Quality Control personnel. With regard to cable protection, four (4) enclosed cable staging areas are being constructed in the plant to provide a suitable controlled environment for cables temporarily stored near the construction activity.

C. Date when full compliance will be achieved:

The Contractor will be in full compliance with regulatory and procedural requirements and construction of the enclosed cable staging areas will be completed by January 31, 1982.

2. Instructions, Procedures, and Drawings The Notice of Violation states, in'part:

" Contrary to the above, Quality Control Inspectors failed to follow procedures, by accepting twelve incorrectly marked (color coded) Class 1E electrical cables. Inspection checklists and Cable Pull Cards were incorrectly accepted for the installed (pulled) cables."

A. Corrective action taken and the results achieved:

Nonconformance Report (NCR's) or Deviation Reports (DR's) had been written reflecting the unacceptable condition of the cables, however notation of these NCR's or DR's was not included on the inspection checklists and the pull cards.

The affected Inspection Checklists and Cable Pull Cards have been annotated to reflect the applicable Nonconformance (NCR) or Deviation (DR)

Report number. Original Inspector signatures on the affected Cable Pull Cards have been voided by lining them out.

B. Corrective action taken to avoid further noncompliance:

The cause of the improper sign-off of the Cable Pull Cards and the lack of NCR/DR identification on these documents was a misunderstanding of proper documentation requirements on the part of the Quality Control inspectors involved.

Consequently, a revision to Quality Control Instruction (QCI) 406 was made which describes in detail the correct documentation and processing requirements to be used by Quality Control personnel when cable pulling activities are
