Dear Mr. Spessard:

This is in response to your Notice of Violation and Inspection Report 50-461/81-32. Illinois Power Company's response to the item of noncompliance cited is as follows:  ;

The Notice of Violation states, in part:

" Contrary to the above, the Contractor was performing cleaning activities on the steam separator (reactor internal) without the use of written procedures or instructions."

A. CORRECTIVE ACTION TAKEN AND THE RESULTS ACHIEVED:

The cleaning activity was stopped immediately when the lack of specific procedural direction was brought to the attention of Quality Control personnel. A Nonconformance Report (NCR 6073) was written to identify the lack of procedural direction and to provide a mechanism for evaluation and disposition of the occurrence. The traveler which should have directed the work (B13-004) was revised to incorporate such direction.

B. CORRECTIVE ACTION TAKEN TO AVOID FURTHER NONCOMPLIANCE:

A Corrective Action Request (CAR 076) has been written to evaluate and correct the underlying cause of-this problem. This has resulted in the determination that responsible supervisory personnel did not recognize that direction of a traveler was necessary to perform cleaning operations in accordance with the established cleaning procedures.

Consequently, the procedures which establish required cleanliness levels, cleaning methods and materials, and general cleanliness acceptance criteria (BA Procedures 3.2.2 and 3.2.6) will be revised to reflect that material cleaning operations must be directed by an established work document such as a traveler or Storage C203170288 820311 PDR ADOCK 05000461 e PDR VEB 2 6 682

f. R.L. Spessard February 22,'1982-NRC Page 2 and Maintenance Instructions and Record card. The methods of directing cleaning operations will be specified in the forthcoming revision of BA Procedures.

As an interim measure, the various Discipline Engineers and Superintendents have been advised by memorandum th.-t all-cleaning operations are to be performed in accordance with written directions.- l C. DATE WHEN FULL COMPLIANCE WILL BE ACHIEVED:

The necessary procedure revision will be issued by March 22, 1982, at which time the contractor will be in-full compliance with regulatory and procedural-requirements.
