This report presents-the results of actions taken by Illinois j Power in response to Supplement 1 of NRC Bulletin 88-10. l Bulletin 88-10-requested that licensees take actions to provide reasonable assurance that molded-case circuit breakers (MCCBs) purchased for use in safety-related applications and without verifiable traceability to the circuit breaker manufacturer (CBM) can perform their safety functions.
Supplement 1 informed licensees that, based on a preliminary .
review of responses to NRC Bulletin No. 88-10, the NRC Staff had determined that some licensees did not adequately address the !
provisions of the Bulletin and that those licensees might, therefore, need to take additional actions. ,
Summary In response to Supplement 1 of NRC Bulletin 88-10, Nuclear Station Engineering Department (NSED) performed a review of the backup material supporting the April 1989, IP response to Bulletin 88-10. Based on the definition of verifiable traceability provided in the Supplement, NSED determined that the material supporting IP's response to the Bulletin did not represent verifiable traceability as currently defined.
To establish verifiable traceability, Quality Assurance (QA) and NSED conducted an audit of the procurement documentation for the safety-related MCCBs in stock which were supplied by Telemecanique. The audit established that 91% of the in-stock, safety-related molded-case circuit breakers-can.be verifiably
. traced.to.the manufacturer. Telemecanique had supplied 827 of tho~907 breakers in stock at the time the review for Bulletin 88-10 was originally performed. (Note that the number of ,
breakers in stock was previously reported as 906. However, a review of the original tabulation and a spot check of the !
inventory indicates that there were 907 breakers in stock at the time that the review was initiated.) All 827 breakers were 1 traced to the supplier, ITE, (see further discusson on page 4) either through a review of purchase orders placed by
.Telemecanique to fill orders for Clinton Power Station or
- Telemecanique purchase orders for stock where Clinton orders were
-filled from Telemecanique stock.
E l Nine MCCBs, furnished by four other suppliers, were removed from L stock because documentation which would establish their traceability to the manufacturer was not obtained from the suppliers.

'Bulletin 88-10 To satisfy the requirements of Bulletin 88-10, licensees were required'to trace all safety-related MCCBs in stock purchased for safety-related applications to the circuit breaker manufacturer.
If'80% or more could be traced to the CBM, the non-traceable breakers were to-be tested in accordance with criteria specified in the Bulletin. If less than 80% were traceable or if more than 1 10% of the untraceable breakers failed the specified tests, the 1 traceability of all installed MCCBs was to be determined and non-traceable MCCBs tested or replaced.
Complying with the Bulletin involved identifying active safety- ,
related MCCB stock codes, the quantity of breakers in stock and the associated purchase orders. Also, copies of the Quality Receipt Inspection Reports (QRIR's) and manufacturer's or vendor's Certificates of Conformance (C of C) for each purchase order for all MCCBs in stock were reviewed. During the process of examining each MCCB to identify the associated purchase order, the breakers were briefly examined for any obvious signs of tampering or refurbishing.
During the investigation, a question arose as to what would be considered adequate evidence to confirm traceability to the CBM.
During a telecon with a Nuclear Regulatory Commission (NRC) representative (P.-Gill and J. D. Weaver on February 7, 1989) IP !
and NRC agreed that a manufacturer's or audited vendor's C of C would be adequate evidence to confirm traceability.
The response to the Bulletin was submitted on the basis that the documentation located in the receiving files established the traceability of all MCCBs.
Bulletin 88-10, Supplement 1 In response to Supplement 1 (clarification of the definition of traceability), NSED performed a review of the documentation supporting the response to the Bulletin. The review indicated that in those cases where the breakers were supplied by other than the CBM, the C of C furnished by the supplier did not establish verifiable traceability, as defined in the Supplement.
Of the 907 safety-related breakers in stock, 838 were purchased from non-manufacturer suppliers and 69 were purchased from manufacturers. Therefore, traceability for 838 MCCBs was the object of IP's. review.
In only one instance where breakers were not purchased from the CBM (Power Conversion Products), was there documentation tracing the breakers back to the manufacturer. This documentation is available because a C of C from the supplier could not be found and the supplier was requested to furnish traceability.

As the result of-the above determination, Quality Assurance-was tasked to audit safety-relatedLMCCBs purchased from other than .
the CBM to determine if they could be traced to the CBM. _Of the !
seven suppliers who provided safety-related.MCCBs_to Clinton Power Station, two are considered to be CBMs (General-Electric-San Jose and Gould)'and five are considered to be non-CBMs
.(Elgar,- Power Systems, Power Conversion Products, Sentry.
Equipment and Telemecanique). The'following discussion details
~
the results of the review of~the.five non-CBM suppliers.
Elcar During a QA auditor's visit to the Elgar facilities, Elgar
- representatives ~ indicated that the documentation needed to establish the traceability of breakers furnished by Elgar to.the' .{
CBM-had been destroyed in a fire. Subsequent to obtaining this-information, three of the four safety-related MCCBs purchased l from Elgar were -removed ' from stock. ~l t
'The fcurth' breaker is one of two breakers purchased on Purchase Order E-98185. The breaker was installed asLa_ replacement for a ;
breaker on the Nuclear System Protection System (NSPS) Division-II static inverter on Maintenance Work Request B06173 in March, (
1985. The breaker is on the 125 VDC supply to the inverter. The breaker is considered to be acceptable despite the fact that verifiable traceability can not be established becaure the breaker,was inspected and tested per General Test Procedure GTP-43 at the time that it was installed and.found to be-acceptable.
In addition,'the breaker application affects only the DC source to the static inverter which is a backup to the AC supply to the NSPS Division II~ loads. ]
Power Systems Because the Power Systems purchase involved only one breaker which was-still in stock, IP decided to remove the breaker from stock rather than perform an audit of the supplier.
' Power Conversion Products
'Three.of the four breakers purchased from Power Conversion Products-were traced to the CBM. However, because a C'of C for each of the three breakers had not been provided~by the supplier, '
Power Conversion provided a letter from Westinghouse indicating the breakers were manufactured by Westinghouse. The fourth breaker was removed from stock rather than perform an audit of the supplier.
Sentry EauiDment The Sentry purchases involved four small breakers which were still in stock. IP decided to remove the breakers from stock
-rather the perform an audit of the Supplier.

Telemecanicue An audit of_ Illinois Power purchase orders with Telemecanique was i conducted on January 8 and 9, 1990 at the Telemecanique facility in Westminister, Maryland. This audit involved reviewing Telemecanique purchase orders for all MCCBs supplied by Telemecanique and currently in stock at Clinton Power Station. <
Prior to 1983, Telemecanique's Westminister operation was part of f Gould/ITE and, therefore, part of the CBM. Up to this time =
breakers were requisitioned for stock with a certificate of conformance.- In 1983, the ITE CBM plants were acquired by. I Siemens and the brand label designation was changed from "ITE" to "Gould/ITE", "ITE/Siemens" and "Siemens". Breaker purchases at the'Westminister facility after this time were changed from <
" stock" to " stock with certificate" and " purchases for a specific account". Certification requirements remained the same. In
-March, 1985, Gould sold the industrial controls division to l Telemecanique, Inc.. Telemecanique has continued to supply and ,'
certify breakers, purchased from ITE. Although traceability to the CBM company has been maintained, the CBM's manufacturing .
location for each breaker has not been recorded. Manufacturing l location can, however, be traced through the data codes on the
~
breakers.
L The traceability of MCCBs Telemecanique supplied to Clinton Power Station was reviewed by auditing Telemecanique purchase orders ,
with the-manufacturer. The Telemecanique purchase orders were i identified by one of two methods. In the first instance, Telemecanique provided computer lists of all Telemecanique purchase orders with ITE or Gould/ITE to identify purchase orders n which either listed Illinois Power or listed a shop order which l
L matched the shop order number provided on Telemecanique's C of C. !
L In the second approach, where a direct match between the IP purchase and a:Telemecanique purchase could not be identified on ,
computer lists, Telemecanique inventory records were reviewed to identify all purchases of MCCDs that were in stock at the time that each Clinton purchase was filled.
The audit of Telemecanique found that all MCCBs supplied to p Clinton' Power Station and currently in stock were purchased from L the CBM, ITE. Attachment C provides a list of the Telemecanjgue purchase orders which were reviewed during the audit of L Telemecanique. This list identifies the Telemecanique shop order, the Illinois Power Purchase Order, the Telemecanique L Purchase order, the manufacturer and model number of the MCCBs and the Telemecanique order quantity. The order quantity shown is sometimes greater than the quantity on the Illinois Power i< order. In these cases, the Telemecanique order u.ay include breakers purchased for other customers or for Telemecanique stock.

Copies of all Illinois Power and Baldwin Associates purchase I order and receiving files reviewed in response to Supplement 1 and all references listed in.this report are retained in the Records-Staging Area under the file subject "NRC Bulletin 88-10, supplement 1 - Backup Material".
Conclusion Actions-taken by Illinois Power have established that all safety-related MCCBs remaining in stock, of those in stock at the time laf the original 88-10 review, can be verifiably traced to'the CBM. All safety-related.MCCBs for which-verifiable traceability was not established during the review have been removed from stock. An audit of breakers supplied by Telemecanique ,
. established that 91% of the safety related molded-case circuit 1 breakers which were in stock at the initiation of the U investigation had verifiable traceability.to the manufacturer.

Nine breakers have been removed from stock because the necessary documentation to establish traceability to the breaker manufacturer was not available or could not be obtained. The application of the one installed breaker which was partlof a 1 purchase which could not be traced has been reviewed and is considered acceptable on the bases that the breaker was tested l when it was installed and that the application involves-only one part of a diverse divisional power source.
1 L The results of the NSED review do not require corrective actions I as required by Criterion XVI of 10 CFR Part 50, Appendix B because IP chose to remove the small quantity of breakers from L
stock rather-than to incur the expense of verification. The fact K that verification was not established does not indicate that a L ' quality related-failure exists in regard to the untraced L breakers. Traceability to this extent was not required prior to the issuance of NRC Bulletin 88-10, and is still not required by regulation.
These actions represent Clinton Power Station's compliance with the requirements of Bulletin 88-10, Supplement 1.
