Dear Mr. Fiorelli:

Pursuant to recent telephone conversations with the Nuclear Regulatory Commission, Region III, the following supplements the Company's letter of December 26, 1979, which addressed the Notice of Violation and Inspection Report No. 50-461/79-11; 50-462/79-01 included in your letter of November 27, 1979. Specifically addressed herein are six points which clarify or amend the original response.

1. United States Testing Compary's Procedure UST-TQ-1, for use at Clinton, is being modified to reflect the education and experience requirements of ANSI N45.2.6-1973 rather than the requirements of the 1978 revision of the standard.

It is anticipated that this change will be issued and in effect by February 15, 1980. Paragraph 3.1.2 of this procedure continues to be strictly administered and forms the basis of qualification for the majority of the civil materials testing personnel, due to the very specific and limited scope of work performed by these individuals. Each level of qualification is to specific ASTM standards and is supported by fully documented training and proficiency evaluations which are directly related to these specific standards.

2. The program for training, evaluation, and testing of civil materials testing personnel is now more formally documented in USTCO's Procedures UST-TQ-1 and QCP-9. To date, the proficiency evaluation has consisted of a practical demon-stration to perform tests to specific ASTM standards, supple-mented by an oral examination administered by a qualified Level III examiner. Either the questions asked in the oral examination will be documented or written examinations will be administered by March 1, 1980.

3. Previous education and experience of personnel hired by U.S. Testing Company are currently verified by existing personnel recruiting and hiring practices. Illinois Power approves each level of qualification for U.S. Testing personnel. Future written requests from the contractor for this approval will contain a statement that applicable education and experience for the individual being qualified have been verified.
4. Qualification documents currently include expiration dates.

A tickler file is maintained to ensure requalification requirements are met in a timely manner.

5. Current practices ensure that all qualification requirements are met prior to certifying testing personnel to any level of qualification. There are no contract arrangements stated or implied which conflict with this practice.
6. Training and qualification records for all U.S. Testing personnel currently performing work at Clinton have been thoroughly reviewed by Illinois Power Quality Assurance.

This review is documented and has verified that all U.S.

Testing personnel have been properly qualified for the testing activities they are assigned to perform.

I trust that our initial letter and the above supplemental paragraphs constitute an acceptable response and, when fully imple-mented, will satisfactorily complete our corrective action.
