LER SUPPLEMENTAL INFORMATION SQRO-50-327/80209 Technical Specification Involved: 4.0.5 Reported 'Jnder Technical Specification: 6.9.1.13.c Date of Occurrence: 9/22/80 Time of Occurrence: 0400 CDT Identification and Description of Occurrence: A continuing review of maintenance requests (KR's) on 5/22/81 discovered that on 9/22/80, maintenance was performed on 1-FCV-70-156. Conditions Prior to Occurrence: Unit 1 in Mode 5 with RCS temperature and pressure at 180 F and 310 psig Apparent Cause of Occurrence: Personnel involved did not consider the maintenance per ormed to affect the

performance of the valve and did not perform the required test. Analysis of Occurrence:' The corrective actions being taken will prevent future occurrences of failure to perform post-maintenance testing. The continuing review of MR's will continue

future occurrences. Corrective Action: Employees have been reinstructed on pre- and post-maintenance testing. A technical instruction is being issued which provides a list of all valves which require pre- and post-maintenance testing along with types of maintenance which require testing.
