LER SUPPLEMENTAL INFORMATION SQRO-50-327/80209 Technical Specification Involved: 4.0.5 Reported 'Jnder Technical Specification: 6.9.1.13.c Date of Occurrence: 9/22/80 Time of Occurrence: 0400 CDT Identification and Description of Occurrence: A continuing review of maintenance requests (KR's) on 5/22/81 discovered that on 9/22/80, maintenance was performed on 1-FCV-70-156. Conditions Prior to Occurrence: Unit 1 in Mode 5 with RCS temperature and pressure at 180 F and 310 psig Apparent Cause of Occurrence: Personnel involved did not consider the maintenance per ormed to affect the

performance of the valve and did not perform the required test. Analysis of Occurrence:' The corrective actions being taken will prevent future occurrences of failure to perform post-maintenance testing. The continuing review of MR's will continue

                  ,to discover events of this type which have occurred in the past. These events will be reported as applicable, but this corrective action should prevent future occurrences.

Corrective Action: Employees have been reinstructed on pre- and post-maintenance testing. A technical instruction is being issued which provides a list of all valves which require pre- and post-maintenance testing along with types of maintenance which require testing. Failure Data: None l l
