- Lack of familiarity with the Fix-it Now (FIN) process CAL Follow Up Inspections 7

Airlock Equalizer Valve Q Actions

- Valve stem is replaced

- Stand-down to train FIN team members on FIN process restrictions

- Limits placed on who can authorize FIN work orders

- Workers not specifically trained in FIN process are not used for FIN work

- Section clock reset

- Training gap analysis to be performed to determine knowledge deficiency
