Dear Mr. Fiorelli:

This is in response to your letter dated June 23, 1980, which included a Notice of Violation and Inspection Report No. 50-461/80-10.

Illinois Power Company's response for the one item of noncompliance cited is as follows:

1. Instructions, Procedures, and Drawings The Notice of Infraction states, in part:

" Contrary to the above, a three-inch hole was cut in the Reactor Building dry wall without approved prescribed procedures,. traveler, or design change."

As confirmed by the inspector who cited this item, the hole in question was drilled in an area directly adjacent to other holes which were being drilled per documented, approved procedures and design. The drilling operation had been verbally authorized by responsible engineering personnel based upon a technical evaluation and was being performed in a controlled manner identical with approved procedures. The infraction resulted because even though the proper technical actions had I been taken, they were not supported by the proper administrative  !

authorization and supporting documentation. When it was dis-covered by supervisory personnel that a problem existed with the drilling operation of the hole in question, the work was stopped and a nonconformance report which fully documented the conditions was written to permit further engineering evaluation.
