o a - p, Tennessee Valley Authority Sequoyah Nuclear Plant LER SUPPLEMEhTAL INFORMATION SQRD-50-327/8004 Technical Specification Involved 3.3.3.10 Reported Under Technical Specification 6.9.1.12.b Date of Occurrence: 3/11/80 Time of Occurrence: 0900 CST Unit 1 Identification and Description of Occurrence: The auxiliary building vent system gaseous effluent monitor sampler flow rate monitor was made inoperable 0900 3/11/80. For the next 10 hours and 20 minutes no flow estimates were performed as required by LCO 3.3.3.10 ACTION 33. (SI 470.5 was not performed.) Portable sampling equipment was installed as required by ACTION 36. Grab samples were being obtained as required by ACTION 34. Conditions Prior to Occurrence: Unit 1 on Mode 6. Action specified in the Technical Specification Surveillance Requirements met due to inoperable equipment. Describe. N/A Apparent Cause of Occurrence: Misunderstanding of Technical Specification requirements. Analysis of Occurrence: There was no hazard to the public health or safety. Corrective Action: All necessary personnel reinstructed as to applicable Technical Specification requirements. Failure Data: l N/A i l l l > 1
