DEFECTS AND NONCOMPLIANCE PART 21 REPORT -IMPROPER DEDICATION OF ACTUATOR PARTSFisher issued an information notice (FIN 2013-03) to alert Palo Verde Nuclear Generating Station of an issueassociated with two valve assemblies supplied to PVNGS by Fisher. Fisher determined that the valves wereprovided with actuator parts that were not properly processed per active valve requirements specified in thepurchase order. Specifically, Fisher provided commercial grade parts instead of commercial grade dedicated items.Arrangements have been made with PVNGS to retrofit the correctly processed parts on the subject valves.

Fisher is informing APS/PVNGS of this circumstance in accordance with Section 21.21(FISHER)

This FIN applies only to the subject equipment supplied by Fisher, identified above, thatwas provided to APSIPVNGS via Fisher Order Number 019-F1 0051845, Items 0001,0002, 0003, 0004,0005 and 0006 (Arizona Public Service PO# 500559374).
Discussion:
Fisher has determined that the subject valves were provided with actuator parts thatwere not properly processed per active valve requirements specified by APS PO#500559374.
Specifically, Fisher provided oommercial grade parts instead ofcommercial grade dedicated items. The parts are identified per Fisher Type III drawingGE55059, find numbers 108, 116, and 139.Action RequiredFisher will perform the disassembly and replacement of theso parts, at no cost toAPSIPVNGS, on Serial Number 20428976 and 20428979 (Tags 3JSGEUVO169 and3JSGEUVO 183) and will support APS/PVNGS in performing a repair of affected, installed valves. Arrangements have been made with APS1PVNGS to retrofit thecorrectly processed parts on the subject serial numbers.In addition, a Corrective Action Request (CAR) has been initiated by Fisher to preventreoccurrence of this issue.10 CFR 21 Implications:
