This report is submitted.in accordance with Technical Specification 3.3.3.8      !
Action (a) which states, "With one or more Loose-Part Detection System          !
channel (s) inoperable for more than 30 days, prepare and submit a Special Report to the Commission pursuant to Specification 6.9.2 within the next 10 days outlining the cause of the malfunction and the plans for restoring the channel (s) to OPERABLE status".
At 0800 CDT on 6/20/91, following a Main Control Board Loose-Part annunciator and performance of an inservice inspection by Instrument and Conti-    technicians and Operations, Channel 3 of the Loose-Part Detection System was determined to be inoperable. The plant was in Mode 1 at 100%
power.
The Channel 3 accelerometer is located in the containment mounted on the        l reactor _ ves;e1 head assembly. Initial troubleshootng of Channel 3 has determined that the problem lies with the sensor mounting or associated sensor cabling. Since the instrument sensor is connected to the reactor vessel head (thus inaccesc:ble during power operations)    further troubleshooting will not tc performed until the next outage of sufficient
:        duration. The remaining eleven channels in this system remaic, operable.
